Hi I am 10 months post op from breast augmentation. I got 415cc high profile implants. I noticed right away that I was getting double bubble on my right breast, and it’s gotten worse as time has passed. My doctor said only solution would be a lift (which he doesn’t recommend) or to tighten the pocket but that gives the illusion of my nipple facing downwards. Is there any other solutions? I hate the way my right breast looks :(.
Answer: Double Bubble Deformity Correction Generally a double bubble deformity occurs when your implant descends lower than the previous breast 'fold' or IMF and the pre-existing fold is still visible (the higher of the two curved lines on the affected breast). This can happen for a variety of different reasons. The best treatment option will partly be influenced by the position of the fold or IMF on the other breast, because achieving or maintaining symmetry with correction of a double bubble deformity is an important consideration.

Answer: Capsulorrhaphy It may be possible to do a capsulorrhaphy, very slight along the lower pole to push the implant up and eliminate the double bubble without affecting your nipple position significantly. An in person exam will help determine if this would be a viable solution that would not involve an implant exchange or other more costly options.

A common one is where the inframammary fold has to be lowered, or the implant drops below the fold. The other is when the cut muscle adheres to the capsule where it places upward traction. In this case there would also be animation, meaning the crease moves up when you flex your pec. This is more likely to be the case when the crease is closer to the nipple than the inframammary fold, as in your case. If so, it requires re-attachment of the muscle with conversion from dual plane to the split muscle plane or subfascial. Muscle release will make it worse.

In most cases, the implant simply slipped too far down the chest wall and traveled behind the breast inframammary crease (the area where the lower breast meets the chest), instead of moving forward to fill the breast cavity. If left untreated, the lopsided implant will form unnatural indentations, resulting in an odd “four-breast effect” rather than the full, rounded chest the patient initially desired. I usually "fix" "double bubble" deformities via 3 methods. 1. Re operation via infra mammary approach to release/cur the tight pectoralis muscle to breasts tissue junction. Fee $4,000. 2. Liposuction of 500 cc fat to fat transfer micro/macro fat to release indented skin. Fee $4,000. 3. Combination of #1 & #2. Fee $7,000. Best to virtual consult with....
