Thank you for your question.Without photos or evaluation, it is difficult to say. After an augmentation and lift, your implants will drop to a more natural position due to gravity. I recommend speaking to your surgeon to discuss your options and concerns.
I would need some photographs or an examination to have a better idea what is going on and why your breasts are still "droopy." It could be that your breast tissue relaxed around the implant or wasn't tightened enough or properly, which could mean a revision lift. Alternatively, it could mean that the implant has lost support and is falling into the bottom of the breast, which needs a different fix. Often, it's a mix of the two. Best to be seen in person to develop a plan.
A revision lift can be performed to improve the ptosis if you still aren't happy with the droop or nipple position.
Unfortunately, you have not provided photographs or told us your bra size. However, it is most likely that your lift procedure was inadequate. The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.
