Hello and thank you for your question. Based on yourphotograph, you may benefit from a tip refining rhinoplasty. Your surgeon canaccomplish this by trimming, suturing, and reshaping the cartilage in yourlower nose. Cartilage grafts may also be used to help improve tip refinement. You may also benefit from an alar base reduction. The most important aspect isto find a surgeon you are comfortable with. With good surgical technique, youcould have an outstanding result. I recommend that you seek consultation with aqualified board-certified plastic surgeon who can evaluate you in person.Best wishes and good luck.Richard G. Reish, M.D.Harvard-trained plastic surgeon
An isolated alar base reduction is good when the base is wide or there is excess flare and the tip is small. An isolated alar base reduction will change the proportion of your nose. In your case, consider having tipwork and alar base reduction together.

Your hanging columella and wide nostrils are due to poor tip support. This can be corrected with local anesthesia and light IV sedation and does not require a general anesthesia. Cost information is provided after an in person visit.
It is possible to do a tip plasty and alar plasty without touching the nasal dorsum. Also, if you want it to augment the nasal dorsum with ear cartilage that can be done. Ear cartilage is used routinely to make many different grafts for nasal reconstruction including the nasal dorsum. ...
