It is possible to do a tip plasty and alar plasty without touching the nasal dorsum.  Also, if you want it to augment the nasal dorsum with ear cartilage that can be done.  Ear cartilage is used routinely to make many different grafts for nasal reconstruction including the nasal dorsum.  Sometimes nasal septum can be used for a straight graft which might work better for the nasal dorsum as opposed to the ear.RegardsDr. J

You could have a rhinoplasty that only affects the tip.  Cartilage can be obtained from your septum or your ear.  It can be used to give your tip more projection and improve the tip aesthetics.  Bridge augmentation is harder but can be done with tip cartilage and some tissue graft. 

All noses widen with smiling.  A tip plasty and alar reduction can be performed without  doing anything to the dorsum.

Yes, it is definitely possible to have a tip alarplasty without changing the dorsum significantly Make sure your surgeon is experienced on this type of surgery, because the balance of the cartilaginous portion of the nose and the bony dorsum must be maintained.
