Will liposuction improve the appearance of my neck angle or should I consider a neck lift? (photos)

Hi! I (F31) wonder what procedures that would be recommended to improve my neck angle? I have always had a bit of excess skin around my jaw and neck but it has gotten worse after some weight loss. Would liposuction alone work or would a neck lift be necessary to get results somewhat similar to the pictures I have attached?

Thank you for your question and sharing your photographs.

The characteristics of the change that you are describing will, in my opinion, not be achievable through liposuction. However, it is potentially possible that an anterior neck lift, which is purely a volume reduction procedure without skin tightening, would still be suitable for you.

The best way to evaluate this would be to have a consultation using 3D imaging and computer simulation. This allows us to measure the surface area of the original shape and of the simulation, and determine whether there is skin excess generated by the volume reduction.

Liposuction alone will not achieve the results you show in the photos.  However a neck lift will although you are generally quite young for this procedure.  There are incisions around the ear but these heal very well.

Liposuction only addresses fat, so it will not do anything for excess skin, which you state is what you can feel and that's also what it looks like in your first photo. I wouldn't recommend for you to do that alone bc you don't look like you really have much fat. Your issue is more likely to be related to either skin and/or poor hyoid position.

While liposuction can remove excess fat, it is not the most effective solution for improving the neck angle because the neck is more than just fat&mdash;it’s a structural system of muscles and skin that change with age. Simply removing fat without addressing the underlying muscle laxity and skin redundancy often leads to an incomplete result, potentially revealing loosened platysma muscles and leaving excess skin behind. 
A neck lift is optimal because it not only removes excess fat when necessary but also repositions and tightens the muscles, restoring proper support to the lower face and neck. Think of it like construction: you can’t just remove weight from a weakened structure; you need to reinforce it from the foundation. Even muscle tightening alone can create a more sculpted shape, proving that the fat itself is not the main cause of a poor neck angle&mdash;it’s the underlying muscle relaxation and lack of support that must be addressed for a truly refined and lasting result.

An in person evaluation is needed to determine skin laxity.  If you have mild to moderate laxity one option is Morpheus8.  This treatment uses RF technology combined with microneedling to improve overall skin tone and texture.  Typically three sessions are recommended. You can add jawline fillers as well to provide further definition.

Liposuction is extremely effective in younger people in an effort to remove unwanted neck fat, whereas neck lifts are usually reserved for those who are older and who have a lot of sagging skin. It is impossible to determine from your photos if you would be a candidate for liposuction or a neck lift. I would instead advise you to meet with a board certified plastic surgeon to determine what is best. I like to caution young people from being too surgery oriented, because there are other issues that contribute to the neck angle, like how the strap muscles are inserted, where your submandibular salivary glands are located, etc. If you have lost a great deal of weight and have been left with definitive loose skin, it would be a contraindication to liposuction, and possibly a neck lift could be appropriate.
