Hello there! I'm sorry you're dealing with a sub-optimal result- that is quite frustrating. Unfortunately, it does appear you will need another revision surgery to smooth everything out. You have an examination by a different surgeon who is known for neck and face lifting. It is likely that both submental incision and ear incisions need to be re-opened to access and undermine the neck completely and smooth out this central neck contour deformity. I would not fall into a "minimally invasive" solution trap. Best of luck.

There are a couple of issues with your neck at this point. The first issue is that I don't think that a neck lift alone is going to remedy the excess skin that you currently have. I think that the proper vector to remove that skin and improve your jawline is up, not back. I feel that you are a facelift candidate and not just an isolated necklift if the first surgeries were performed correctly. The second issue is the scarring and skin quality. I think release of the skin from the underlying muscle needs to be performed. Exploration of the status of the digastric muscles should be evaluated as I do not think this is a gland issue. Fat grafting may need to be performed after the skin is redraped and the underlying platysma and digastric muscles are corrected.

Based off your photos - it appears you have central banding, skin excess submental, some slight jowling - your old submental scar is noted as well. Potentially need to redo the neck lift (not sure what was done the first time) but it seems the excess skin, bands, laxity were not addressed at the first operation. You have some slight jowls forming - that could benefit from full face lift to rejuvenate the midface + lower face + redo neck. If addressing all of these components as a whole, it will allow for the result of a more youthful appearance to be fitting and more natural. Seek out your surgeon to discuss a redo neck lift and other surgeons for second opinions. Ideally the submental incision used again to fix the mid central neck, plicate the muscle, and likely a behind the ear incision to give your lateral neck / mandible a more defined appearance with removing excess skin in that vector. Some fat grafting to the mid face + / - any other issues you would want to address by incorporating a mid / lower face lift - endoscopic versus open. All things to discuss with your surgeon(s). (If you are planning on doing something surgical, stopping external skin tightening modalities will limit post-op issues ie. Morpheus, etc).
