I went to Dr. Schoenfeld for rhinoplasty in April of 2021. I consulted with several surgeons in the DMV and tri-state area and ultimately chose Dr. Schoenfeld for many reasons - he was straightforward in our discussion about what was and wasn't achievable, he listens to what you are looking for, and his personality is so warm and puts you at ease. I had a rhinoplasty done several years ago that was NOTHING like I had hoped. The doctor was focused on everything BUT what I wanted, which was simply a tip reduction. He pretty much butchered my nose and the tip was droopy as a result. Dr. Schoenfeld immediately knew what he wanted to do to fix my nose and had a detailed plan for it. My nose looks restored and natural now and the tip looks perfect! It’s so nice to look at pictures of myself and not even notice my nose, which has never been the case. It’s so subtle and it looks gorgeous and natural.
I had deep, dark circles and bags under my eyes that no amount of filler or concealer could help. In fact, filler and concealer made them look worse! Dr. Schoenfeld recommended a transconjunctival (inside the lid) lower blepharoplasty with fat repositioning. Other surgeons I consulted suggested also doing a skin "pinch" at the outside corner as well as some fat grafting. I went with Dr. Schoenfeld because I liked his more conservative approach, and I couldn't be happier with my results. The thing that most astonished me was the easy recovery. I had absolutely no pain, and the bruising and swelling resolved very quickly. Before surgery, whenever I looked in the mirror, all I could focus on was how sad and tired I looked. After surgery, I still look like myself, but a much better-rested, younger version. Dr. Schoenfeld is the best -- warm, honest, funny, and super smart. He totally knows what he is doing. I did my homework when I was deciding on a doctor for my Rhinoplasty/Revision Septoplasty. I went to six or seven consultations in the DC area. Dr. Schoenfeld's digital mockup of my nose and his personality won me over. I had the surgery done in October 2020 and I LOVE my results. Dr. Schoenfeld was able to refine my nose naturally and I can breathe normally now. During the consult, I asked for the following (cosmetically): * A small hump on my nose to be taken down * The tip refined and lifted slightly, and a slight * Added projection * My nostrils brought in slightly I ended up with exactly what I wanted: more feminine but still ethnically congruent as an Indian woman
Dr. Schoenfeld really has an eye for what is going to look best on his patients. I went to him for lip fillers a few months before my wedding. I wanted everything to look natural but still noticeable on some level. I kept getting compliments from people saying “You look so good. Something is different about you but I just can’t tell what”. Had buccal fat removal and jowls/neck laser lipo. Very pleased with the results (after 2 weeks). Dr. Schoenfeld is very charismatic and honest, he immediately identified the necessary procedure and delivered the best results.
From the photos you’ve provided you can absolutely have the chin and nose that you have been playing with in your own images. I might suggest a slightly less pronounced chin as this seems to overtake your lower face a bit. It could be disgusting during a consultation, using morphine. Most importantly, the morphing of the images should be completely transparent, and your surgeon should simply convey you that these are morph images.
From the photos you have provided, it appears that although there is a hint of the inverted V deformity that is not the issue that seems to be taking place. Some of the views that you were provided show a very nice result, but the one view from above shows significant dorsal deviation primarily of the cartilagenous portion of the nose. This likely occurred overtime due to scar contracture. None of the issues you’re having with your nose, however, impossible to fix and if that’s something you’re interested in then you should pursue this with an experienced revision rhinoplasty surgeon.
Not all revision rhinoplasty surgeries are the same. Based on the photos that you have provided, it appears that you would be an excellent candidate for some revision work on your nose. How that revision work is performed, either open or closed is not really the issue, but which approach your surgeon is most comfortable with should dictate that approach.
In the past, obstructive sleep apnea would be a huge red flag for elective cosmetic surgery. BMI (body mass index) and overall health of the individual are now looked at more closely in terms of safety for elective cosmetic surgical procedures and general anesthesia. You will need a medical clearance prior to any procedure, but if you are otherwise healthy then OSA is not a contraindication for general anesthesia or IV sedation. You should also be operated on in a place (hospital or outpatient center) that has a board certified anesthesiologist available. On another note, if you have lost 140 lbs, then you should retake your sleep study as your apnea may have decreased enough that you are no longer considered high risk.
If you are referring to your lower eyelids then you can try Botox to the orbicularis muscle to reduce the "roll" effect that is occurring when you smile. Trimming the orbicularis muscle is a surgical alternative. Upper eyelids would require surgery and there is really no substitute.
