Would liposuction help minimize my bra rolls or should I consider a bra line back lift? 60 y/o in good health. (photos)

Back and lateral torso are totally Independent anatomical areas and must be treated differently.
You have excerpt skin on your back which means excess of length and the Back lift and liposuction will be both great procedures to create a thinner and smoother back.
Lateral torso is the sides of breast and anterior to back, you have excess of fat, and also doble vector of excess of skin: excess of length and excess of width. So you will need and inverted “T” incision to remove both.
The end result incision will be a 270 incision which starts underneath the inframammary fold all the way around towards the other side and a vertical incision both sides on The lateral chest.

Hello, from the photos if it seems that you require liposuction of the arms, torso and back, and a cut in the bra line in the back but it does not seem that circular is required, with a cut in the middle of the back is enough to produce the lifting and improve the shape

Thank you for your question regarding whether you should opt for a bra line back lift or liposuction. Based on your pictures and age, I believe a procedure involving an incision, like a back lift, would be the most beneficial as your skin may not spring back effectively post-liposuction.

There are two types of back lifts you can consider: vertical and horizontal. A vertical back lift involves an incision from the armpit down the side of the chest, limiting the visibility of the skin rolls seen in your pictures. The benefit of this procedure is the relatively concealed scar.

On the other hand, the bra line back lift, also known as a horizontal torsoplasty, involves an incision from one outer breast corner, across the back, to the other outer breast corner. Ideally, the scar from this procedure is concealed under the bra strap, hence its name.

The bra line back lift will give you the best results. Due to your age, the elasticity of your skin will likely prevent any significant skin retraction following liposuction. Therefore, a bra-line excision will be the best way to remove the excess tissue.

Thank-you for your question. You do have skin laxity in the back and would be better off with a back lift. There will, of course, be scars in the bra line. Another alternative, though not as effective, would be a ThermiTight or Bodytite procedure, which does not leave any scars. You need a combination of upper body lift/bra line lift and liposuction. If you have liposuction alone, it will deflate your skin and make it more droopy.

An in person evaluation is always best, but your photos indicate that your concerns are caused by a combination of fat and skin laxity. The liposuction would address the excess fat but may leave you with contours that you won't like. The bra line lift will address both.

The bra roll area can be somewhat tough to treat. If there is good skin elasticity/tone, then sometimes liposuction of the upper back/bra rolls along with skin tightening (such as BodyTite, J-plasma or similar) is sufficient to decrease bulk in these areas and tighten the back skin. In patients that are a little older, have more lax skin, or have lost significant weight, it is generally recommended to consider a backlift. This is sometimes called an upper body lift. Essentially, it removes extra skin and tissue to pull up the redundancy of the upper and mid-back. It results in a scar that is typically placed in the bra line but extends across the back from side to side. If there is extra skin under the arms, it can be extended there to the breast crease. The nice aspect of this surgery is that the tissue is removed and the smooth and tight appearance afterward is readily apparent. As opposed to liposuction and skin tightening which can take 3+ months to know how much effect it will have and still may not achieve what you are looking for. It's similar to a tummy tuck. The tradeoff is skin/tissue removal, but it results in a long scar. So it comes down to what you are most okay with and your aesthetic goals. Hope that helps! Wish you all the best!

Bra rolls can be composed of fat, lax skin, or a combination of both, as it appears to be in your situation. If you are at a healthy stable body weight (with a BMI preferably less than 30), consider an upper body lift, accepting the transverse scar within the upper back bra-line. Alternatively, a liposuction will lessen the amount of fat present, but will alter skin laxity to a variable degree depending upon previous skin damage (stretch marks).
