Dr. Fletcher grew up in the Pacific Northwest. He earned his undergraduate degree in Health Science at Walla Walla University and his M.D. degree at the University of Washington School of Medicine. While at UPMC, Dr. Fletcher learned from some of the very best surgeon educators in the country. During his research year, he served as the Hand Transplant Fellow, taking part in the first bilateral female hand transplants performed in the United States. While at UPMC, Dr. Fletcher became obsessed with the mastery of surgical technique driven by the desire to offer his patients the very best care he possibly could. To that end, he sought out additional training by way of the following subspecialty fellowships: Facial Plastic Surgery Fellowship. Dr. Fletcher became fascinated with facial cosmetic surgery while in residency. This interest drove him to seek out additional training with the late Dr. Mark Codner. Dr. Codner was a master of facial cosmetic surgery. His books, articles and lectures on eyelid surgery, facelifts, and rhinoplasty have been critical to the advancement of facial cosmetic surgery around the world. While under the tutelage of Dr. Codner, Dr. Fletcher learned advanced techniques in upper and lower eyelid surgery, face and neck lift techniques and rhinoplasty surgery. The University of Pennsylvania Microsurgery Program. This training experience focused on advanced techniques in reconstructive microsurgery with an emphasis on breast reconstruction after breast cancer. Because it is among the busiest microsurgery programs in the world, fellows are able to partake in hundreds of complex operations, gaining vast experience in a short amount of time. Dr. Fletcher refined the skill of small blood vessel and nerve dissection necessary for perforator flaps like the DIEP, SIEA, SGAP, IGAP, and PAP. This skill enables the surgeon to harvest tissue for breast reconstruction while minimizing injury to the patient’s body. In addition, he learned how to shape this tissue into a beautiful, natural-appearing new breast. At Penn, Dr. Fletcher discovered his passion for helping women recover from breast cancer through reconstructive surgery. Because of this training experience, he is able to offer advanced and comprehensive care to his patients. The Curtis National Hand Fellowship. This fellowship is known for its dedication to outstanding surgical education. At Curtis, Dr. Fletcher learned advanced techniques in fracture care, tendon reconstruction, arthroscopic surgery, microvascular surgery, nerve repair and transfer as well as reconstructive bone and soft tissue surgery. Dr. Fletcher is committed to providing the highest level of surgical care to his patients. My experience went above and beyond my expectations. All I could think of was getting smaller breasts to improve my daily function. I kept telling him that I was not concerned about what my breast looked like as long as they were smaller “ no offense to anyone that was forced to have them removed. I have been larger than a c cup since third grade and have built a distain and not great relationship with the one I had”. After losing 160lbs I wanted the extra skin removed. I came to Dr Fletcher after speaking with two other surgeons. After my first appointment with him I knew he was the provider I wanted performing my surgery. Dr Fletcher has a fantastic bedside manner and did a wonderful job of laying out all the information on my surgery, the benefits, risks, and assessing realistic expectations. I am a rather anxious patient and I always come into my appointments with more questions than I know what to do with. Dr Fletcher always kindly, calmly and consistently answers all of my questions, calming my nerves. The surgery itself I had done was a circumferential body lift, repair of my abdominal muscles and liposuction. My scar is so well placed and as pretty as a scar can be, mine definitely is. It is flat, small, and low enough that my underwear cover it. To say Dr Fletcher is skilled or talented is a complete understatement! I trusted him going in but my results are above and beyond what I was expecting. I am almost 10 weeks post op and I’d describe his talent as hands of gold holding a scalpel. Dr. Fletcher was my surgeon for 2 procedures. The first procedure was a bilateral mastectomy (performed by the surgical oncologist.) At this time, Dr. Fletcher put in expanders. The second procedure was for breast reconstruction. In both cases Dr. Fletcher was thoughtful, caring and provided me a ton of information. Dr. Derek Fletcher was amazing, thorough, supportive and friendly throughout my journey. During my pre-op appointment he took the time with me to discuss the look I'm trying to aim for as of measurements and fullness. I settled for Gummy Bear (Natrelle) implants, Moderate profile, 380CC on both breasts. Before, I never wore a bra and I swore by sports bras or bralettes. Now, I measure at 36D. For any ladies wondering, this all costs around 7.7k. Surgery day was nerve racking but he and his team made me feel comfortable about going under. The pain I've had were the recovery days. Rest, rest and rest were all I did for 2 days. It was hard getting up and using the bathroom. But after that, I didn't need the medications. I healed great. My incisions also healed nicely. Dr. orders to not touch the incisions and avoid sweat around the area, let them heal on their own. I saw him after the day of surgery, 2 weeks & 6 weeks to see how I'm doing and how my breasts were healing. On my 2 month appointment, he let me know I can go back to doing cardio, swimming in lakes and wearing under wire bras. Worth every penny and patience. I'm happy about the look I've achieved.
No complaints whatsoever. Dr. Fletcher was kind and attentive and did great work. There has been no point where I didn’t feel heard or that I would be taken care of. He even gave his personal number to contact him and offers revisions for up to a year after! Still healing but so far I have beautiful results.
A well executed facelift that produces lasting results (10 years or more) must address all the key deep structures of the face that contribute to facial aging. These procedures are time tested, reliable, and can achieve beautiful, long lasting results. They re-suspend the supporting structures of the face, address the deep structures of the neck, and restore balanced facial volume. Deep plane lifts are not "mini lifts," "lifestyle lifts," or "3 day lifts." They require skill and intimate anatomical knowledge on the part of the surgeon. In addition, they required commitment to a real recovery on the part of the patient. Over the last 30 years, there have been numerous "minimally invasive," "fast recovery" procedures promoted by many surgeons. Some of the techniques can help patients in their 30s and 40s age more gracefully. None of them have been shown to reliably provide lasting results to the average 60 year-old. They can't provide lasting results because they intentionally do not perform all the maneuvers necessary to do so. Results from these procedures in your age group often do not last more than a few months. Correctly performed facelifts are not skin tightening procedures. Be weary of anyone who suggests otherwise.
Unfortunately, I think you are correct on both counts. The photos definitely confirm inferior migration of our implants and strongly suggest failure of the rectus plication. Both will eventually need to be surgically corrected.
I would start with an in person exam to assess your soft tissues and abdominal wall. I would likely get an ultrasound to rule out a seroma. If no seroma was identified, and your physical exam suggested an abdominal wall problem, I would consider a CT scan to assess your abdominal wall or explore with surgery.
It depends on the thickness and density of your breast tissue where it blends with your chest along the upper breast boarder. Generally speaking, if your pinch test in this location produces a skin-to-skin thickness of 2 cm or more, you are a candidate for breast augmentation on top of the muscle. Sientra textured anatomic implants work exceptionally well for women with your breast shape when placed in the sub facial position (under the connective tissue on top of the muscle). Textured implants may not be as safe as smooth implants. You should have a detailed discussion with your surgeon about the merits of each implant type before making your decision. Best of luck!
