After years of yo-yo dieting and eating disorders my breasts are saggy, can a surgeon manipulate the tissue back into shape?

I recommend a breast lift with implants. Since you are looking for upper pole fullness, in addition to repositioning of your current breast tissue, you may want to explore the options of doing a breast implant along with the breast lift/mastopexy.  The breast implant will give you that upper pole fullness you are looking for. 

It sounds like you should be considering a breast lift (mastopexy). This will lift, tighten, and reshape your breasts. An implant can be used to enlarge your breasts, make them look rounder and create more upper breast fullness. If you are an E cup you have quite a bit of breast tissue so an implant may not be necessary. A consultation is necessary to determine what is the most appropriate surgical plan for you. If you have enough breast tissue, a mastopexy, or breast lift, can restore a very attractive and youthful breast shape. To obtain a very full and round upper pole, you’ll likely need to combine breast augmentation with a breast lift. The implants can provide a greater degree of fullness in the upper pole, Good luck!

If your breasts are droopy, a lift can be performed to get them higher and perkier.  Often times an implant is also added to obtain more upper pole fullness.  However the results are not the same as a patient who only undergoes an augementation because of skin elasticity.  Usually the breasts are droopy because the skin has lost its strength and cannot hold up the breasts so even after a lift, the skin will drop and they will look a little more natural.  It's very difficult to obtain a round shape when the skin has lost its strength.

Regardless of technique, loss of upper pole fullness is inevitable, especially with larger breasts. One would think otherwise, that adequate breast tissue can re-volumize the upper pole, but instead its mass leads to greater and quicker lower pole expansion, and drop out of tissue. The use of moderate sized, moderate profile (not high profile) implants can give reliable and long term upper pole fullness to breast lift and reduction patients. Alternatively, fat grafting can also be effective.  Go visit a few ABPS certified/ASAPS member surgeons who specialize in cosmetic breast surgery. Best of luck!

You are right about loss of fullness in the upper pole in most patients after breast lift. Due to gravity breast tissue will start sagging few weeks after surgery and fill the lower pole, leaving upper pole flat. Nipple and areola will not travel south and patient will never have the same saggy look like before surgery, but upper pole will become flatter in time. In order to compensate for this shortcoming of breast lift (mastopexy) we use implants which are usually placed in submuscular plane. This gives longevity to a breast lift result keeping upper pole fullness for ever since implants do not respond to gravity. We do these two procedures (augmentation and lift) in the same time. Sometimes, we do a small breast tissue reduction to decrease amount of breast tissue in the lower pole and late sagging.
