Corset platysmaplasty was performed as part of full facelift (also incisions in front and behind ears). A year later could still feel hard, ropy, lumpy vertical cord in midline - as if it's the ridged stitching together of the platysma. Why would this have happened (I can't find other complaints about this issue online)? Looks worse than before any surgery, as it was only loose skin before with no visible bands. Why would it be hard and not soften over time? How would it be corrected? Thank you.

Answer: Ridge You are likely feeling the suture line from your platysmal plication. This can be released but only surgically. Alternatively Botox can often be used to soften the band and improve your appearance.

Answer: Raised ridge after neck plasty and facelfit. This can occur after the procedure you describe. Its difficult to answer without seeing images. Likely you are seeing the edge of the platsyma plasty which can form a ridge in the area described. If you are thin or if excess liposuction was performed you may see the edges of the muscle plication. This may settle with time. If it does not resolve a revision procedure though a limited submental incision is generally what is required.

As you have surmised, this is likely related to the suturing and reapproximation of the edges of the platysma muscle in the neck. It may be attributable to the tension of the repair, scarring at the level of muscle, or it may actually be due to the type of sutures employed. It is likely that this can be improved through a revision, and I would encourage consultation with your operative surgeon.

It may be residual scar tissue that has formed the hard cord you describe or it may that your platysma muscle was over tightened causing the ridging you describe.It is possible to correct this however I would suggest seeing a Plastic surgeon who can examine you fully.I hope this helps and sending you the best of luck
