Do I need a full revision? (Photo)

Hello im Tony a 20 year old male. I started with my max weight of 210 lbs and I went down to 140 lbs. I noticed that I had loose skin around my belly and so I decided to get a full tummy tuck with lipo to my flanks. I’m now 7 months out and have maintained my weight of 140 lbs and I am not too happy with the results as you can see with the photos that I am going to provide my flanks have drastically increased in size and my right abdomen protrudes more than my left. How can I adress this issue?

From your photos, it looks like you’ve achieved good results overall, and congratulations on your significant weight loss. If you're unhappy with residual fat in your flanks and along the superior iliac crest, additional targeted liposuction in these areas could help provide a more refined contour.

Further liposuction in the abdominal area may also be beneficial to enhance your results by reducing any remaining fat deposits, creating a more even appearance. This targeted approach can help balance any asymmetry, such as the difference in protrusion you mentioned between the right and left sides.

A full revision may not be necessary, but consulting with your plastic surgeon about a smaller, focused liposuction procedure could be a great next step. I hope this helps guide you toward your ideal outcome!

Congrats on the weight loss! Based on your pictures, it does appear that you still have some loose skin of the lower abdomen. Additionally, there is a little bit too much fullness of the skin just above your incision. I see this really commonly, and I generally recommend liposuction in the abdomen and revising the scar. This should leave you flat and firm.

The protrusion in the side areas is usually due to the pelvis bone being outwardly positioned. If there is no noticeable fat tissue when palpated, unfortunately, there is nothing that can be done about it. However, if fat is present, liposuction can be performed. For areas with excess fat in the abdomen, liposuction can also be used to remove fat and correct any asymmetry.

Your overall shape and contour is good. Liposuction of your flanks and your abdomen can help to thin that tissue, so it’s thickness matches better with the skin which is below your scar. Then a mini tummy tuck/scar revision of your abdominoplasty scar should help with the small amount of loose skin that is above your scar. Congratulations on your impressive weight loss!  From the submitted photos it does not appear that you need a full revision.  You should have a nice improvement with some liposuction to the flanks to give you more of the result you are wanting.

I would suggest doing liposuction now for the excess fat. Liposuction is not done in the area of the tuck with the tuck for safety to the skin. However, it works well to do it prior or after the tuck to remove as much fat as possible.
