Facial balancing, rhinoplasty - Options? (photo)

Question

What would you recommend to help balance my face? I currently have excess skin under my eyes, hollow temples and extra volume in the lower half of my face. Also wondering if rhinoplasty would be able to help with the width of my nose, which appears worse when smiling.

It sounds like your nose is your primary concern, so let's focus on that for now. Addressing the shape of your nose (especially the nasal tip) is actually one of the most common reasons people seek out and undergo a rhinoplasty. An open septorhinoplasty can best address both your aesthetic concerns and any functional issues with your nose. I would be able to adjust the size, shape, and definition of your nose (particularly the bridge and tip)!

This truly is a surgery of millimeters- and whether you're looking for a minor adjustment or a major transformation, it requires exceptional surgical skill to achieve functional and natural-looking results! Because your nose is the DEFINING feature of your face- I would only recommend seeing a surgeon who specializes in rhinoplasty procedures, and who has significant training and experience performing these surgeries. I would recommend a lower eyelid blepharoplasty to address the lower eyelid excess skin. This can be combined with laser to treat the skin "crepeyness". I would recommend a rhinoplasty if you are seeking one for nasal shape and/or breathing purposes.

Based on your photos, you are a good candidate for rhinoplasty. The tip droop can be brought up and the tip and bridge can both be made a bit thinner. At the same time, the nose would be supported better so it resists drooping when smiling. Fat transfer can easily address hollowing temples and areas around the eyes and cheeks. Be sure to see a rhinoplasty expert for consultation.

I would recommend a mini lift with high SMAS technique that would lift your cheeks, reduce nasolabial folds and reduce temporal hollowing.  A short scar technique can be used if you do not need any neck tightening.  A rhinoplasty can be added to refine your tip and narrow your bridge.

I can see that you have a bit of a wide and asymmetrical and droopy nasal tip appearance. If this is bothering you a lot, rhinoplasty surgery may be considered for achieving a narrower, more symmetrical, and less droopy nasal appearance. Fillers may be considered for your temple hollows. If you draw up a vertical line from your medial canthus, your nose should be the width of that or smaller. You nose appears to be normal width. Your tip could be refined as well as the dorsum and increase the nasal columella angle. However, another thing to consider would be a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. You would also benefit from Botox to the crow's feet areas.

It would be very difficult to suggest how to bring more balance to your face without in person or at least teleconference consultation. It is quite possible that cheek augmentation with fat grafting and lower blepharoplasty would address some of the issues, possibly conservative rhinoplasty. However, in person consultation with a board certified plastic surgeon with expertise in facial rejuvenation would be my recommendation. Good luck.
