Button, bulbous-like nose. Can I do a closed rhinoplasty? (photo)

Question

Hi, I’m a 33 year old female interested in getting a rhinoplasty to reduce the size/width of my tip, not change the shape, and make my nostrils slight less flared and more symmetrical. My nose is a button, bulbous-like nose. I absolutely don’t want to touch the bridge or the swoop. It is perfect to me and most people get surgery to make their nose profiles look exactly like mine. Is it possible to do this with a closed rhinoplasty?

Hi, It is better for you to consult with your facial plastic surgeon who can examine you and give you opinion based on examination. it is possible to do it either way open or closed depend on your surgeon skills.

As far as I can understand from your question Closed rhinoplasty operation does not offer permanent solutions as much as open rhinoplasty. In open rhinoplasty operation, your surgeon's field of vision will be open and will be able to intervene in every area, so I recommend open rhinoplasty operation.

Tip plasty maybe performed as an isolated procedure to provide a narrow nasal tip appearance. That said, you appear to have a pleasing nasal appearance on front and profile view. We do not perform any rhinoplasty surgeries without an external incision, especially when tip work is being performed, since this meticulous work needs to have full visualization. Hope this helps! You can achieve a conservative natural looking change of the tip and nostrils easily with a closed tip-plasty and alarplasty. 
Tip-plasty is a great procedure to address a bulbous tip only and leave the rest of the nose alone. It can be accomplished through an open or closed approach. The goal is to refine the tip and make it smaller and smoother. 
In tips with strong cartilages but thin skin, all that is needed is removing some of the cartilage and softening the rest by gently crushing it (morselizing). 
In tips with thick skin and lot of soft tissue or fat, the excess fat and soft tissue is removed along with removing some of the cartilage.

Hello and thank you for your question. Based on your photographs, you may benefit from a tip refining rhinoplasty. Your surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. This can be achieved with either closed or open rhinoplasty. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.
