Curved nose when I smile - Droopy tip? (photo)

Question

I don’t like that my nose, when I smile, curves. It’s not nearly as evident when I have a neutral face, is that a drooping tip? There’s also a pronounced long/sharp bone (or cartilage?) that ends and gives me a bump right above my tip. Is that a dorsal hump even though it’s much lower than any pics I’ve seen. Will rhino change what my nose looks like straight on? It looks almost like a triangle. Also should I reduce the overall projection of the nose?

I would recommend a dorsal hump reduction and lift the tip so that the caudal septal angle (pronounced bone) is not visible.

As far as I can see from your photos you have a dorsal hump, it is possible to get a more elegant nose that is compatible with your face with a single operation, for this I recommend you rhinoplasty operation.

The arrow in your photo is at the bottom of your cartilaginous nasal bridge. When you smile, your tip droops, and this bridge sticks out a bit too far. It is referred to as a poly beak. tip dropping when you smile: this is due to a muscle we all have in our faces, but yes it can be a "drooping tip" as you call it. The sharp piece of cartilage before your tip is the end of your septum. This is not called a dorsal hump. Rhino can change whatever you want from the side or front and address projection.

Based on your photos, you are a great candidate for rhinoplasty. The bump can be taken down, and the length of the nose reduced. Then the tip cartilages can be reshaped and lifted some so it won't look droopy at the end and have better support. It should give you a great result that looks natural. I think you would benefit greatly from an in person consultation with a plastic surgeon focused on rhinoplasty.
Based on the photos you’ve provided, you have an under rotated tip which dynamically worsens when smiling due to a muscle that pulls it down. This is not uncommon. Also, because your tip is under rotated rather than sitting at the anterior septal angle, the anterior septal angle is more prominent and pushing up to create a “hump.”

You would benefit from a rhinoplasty and you may also want to consider a chin implant to balance your face. Your nose is too large, you have a dorsal hump and the nasal columella angle is 90 degrees and should be 110 degrees.

Thank you for your question. Rhinoplasty definitely can help with drooping tips-especially when smiling. I think it is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. You should be able to have a great result.
