Based on your photos, you appear to have medium thickness skin of your nose, along with a hanging collar, and a wide appearance of your tip and nostrils. If you do not have a dorsal hump, it is conceivable that tip plasty, and alarplasty the may be considered for achieving a lifted and narrowed appearance to the lower one third of your nose. Yes, it is absolutely possible to only perform a tip rhinoplasty to improve the shape and contour of the nasal tip. It would be similar in terms of recovery as an open rhinoplasty. It is important to thoroughly discuss expectations with your plastic surgeon for rhinoplasty and specifically the parts of the tip that bother you so a specific surgical plan can be created. A tip plasty will only allow access to the very tip of the nose. You need the base of the nose repaired as well and will include the crural feet. You are better off with an open rhinoplasty. You may also consider a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners.
