I had a Facelifft / Midface lift and Neck lift - 6 weeks ago and was hoping that the horizontal wrinkle along my left jaw line would go away. My nabisbosal folds seems to also have come back. Is this something that can be corrected to get rid of the obvious line on my left jawline? My doctor said he put fat grafting in that area but it didn't hold. I also had dermabrasion around my mouth area but is now bumpy in texture.

The horizontal line will unfortunately not be eliminated by a facelift, and the partial recurrence of the nasolabial folds is due to the inevitable skin stretching and relaxation that occurs over the first several weeks after surgery. Both can be improved by additional fat transfer or filler, and the horizontal line could also be improved by selective dermabrasion or laser to that wrinkle. A facelift really is best at addressing fallen cheeks and jowls and a loose neck, but it is much less successful in improving wrinkle lines, which require a resurfacing procedure such as laser, dermabrasion, or a chemical peel. Answer: Laser + Fillers It appears, from the photos you provided, that you could benefit from CO2 fractional ablative laser skin resurfacing and dermal fillers. I often explain CO2 ablative laser skin resurfacing to my patients using this analogy: If you have a wrinkled tablecloth and you pull it tight (with a surgical lift), it will look better, but the wrinkles could still benefit from an iron (CO2 Laser) to really make the tablecloth look smooth and pretty. CO2 Fractional Ablative Laser Skin Resurfacing also helps with skin texture, pigmentation, and sun damage. However, I typically recommend my patients do ablative laser resurfacing at the same time as their surgery, because the recovery is significant. If you are unwilling to undergo another recovery period of a week or more, then you could research a series of fractional erbium laser skin resurfacing treatments, such as Fraxel Re:Store Dual, or a single, fractionated CO2 Laser treatment, such as Fraxel Re:Store. I recommend dermal fillers as an alternative to fat transfer for plumping up those nasolabial folds, since fat transfer did not work for you previously. In searching for a provider for Lasers and Fillers, you should ask the provider about their experience with these specific treatments, and look for someone board certified in cosmetic surgery or plastic surgery. Sometimes, plastic surgeons are only surgeons, and sometimes they’re also expert laser providers. These are different skill sets. Be sure to ask about the variety of lasers and fillers they offer, experience, and certifications or awards. A diamond-level injector award from Galderma or Allergan (the two major manufacturers of dermal fillers) is a good indicator of experience with fillers.
