Thank you for your question. There are different options to improve your buttocks: implants, buttocks lift (skin removal), or BBL (fat transfer). I would recommend first doing a buttocks lift to remove excess skin followed by a fat transfer to augment the volume. If you tried adding volume now to the loose skin the results would not look natural. Either one would be recommended based on the sagginess, lack of volume, and your goals. You can also combine some of these surgeries.

A circumferential lower body lift and gluteal fat transfer may be the best option for you. The lower body lift can tighten the upper quadrants of your buttocks and the fat can add volume. This combination of procedures will not be able to tighten the wrinkles at the level of the gluteal creases.

Hello @lkp752, thank you for your question. A rapid and great loss of weight can cause the skin to become really flaccid. What gives structure to our body is not only the muscles or bone, but the fat and when it start decreasing really rapid skin start to sag. There are different ways to approach this sagging, such as a lift and reconstruction of the gluteal sulcus, is really important to create a surgical plan that meets your needs, meets your expectations, and takes into account what is realistically possible. Loss of buttock projection and skin laxity of upper thighs are dominant features of your problem. This could be improved by means of buttock/thigh lift, buttock augmentation with flaps and fat grafting. In person consultation with a board certified plastic surgeon with expertise in these procedures, is recommended. Good luck.

You only have a few options. First, no surgery (of course), second, a BBL (remove fat from almost any undesired areas and fill the deflated buttocks), third, an implant and fourth, a buttock lift by excising excess skin (leaves substantial scars.

Generally the only realistic options for gluteal augmentation are implants or fat transfer aka BBL.
Implants have high complication rates and fairly low long term patient satisfaction rates. Most plastic surgeons do not offer gluteal implants for good reason.
Weight loss patients generally have skin laxity that is a major contributing factor. Skin laxity contributes to contour and lessens what can be achieved with both liposuction and fat transfer. You should take this into account when doing consultations. Procedures that work well on tight young women may not work as well in your case. Doing skin laxity surgery and fat transfer may be an option but most people are looking for a reasonable solution. having things lifts followed by a BBL may be more than some are willing to go through considering the results will still only me a moderate improvement. Your skin laxity lowers your candidacy and you should take that into consideration.
To get a better understanding of your candidacy and treatment options I suggest having multiple in person consultations with local board-certified plastic surgeons.
If providers recommend certain procedures then you should insist on being shown a sufficient number of before and after pictures to get a clear understanding of what those procedures will entail including a good understanding of what the procedures can and cannot accomplished based on your individual candidacy for the procedure.
Liposuction and fat transfer results are highly dependent on individual peoples candidacy for quality outcomes.
Skin laxity is one of the major reasons patients fail to be exceptional candidates.
This does not mean that you can’t have it procedures done but you need to get a good understanding of what can and cannot be accomplished with each procedure based on your own candidacy.
To some degree this depends on what you’re trying to achieve and what you’re willing to go through.
There’s no single straightforward great procedure for what you’re trying to achieve.
A BBL procedure will be inherently limited an individual Who have flabby loose skin.Ask providers to open up your portfolio and show you their entire collection of before and after pictures before committing to having any surgical procedure. Ask providers to show you all the results of previous patients that had similar candidacy for the procedure that you do. This means the same amount of skin laxity, overall size, amount of body fat etc. etc.
An experience provider should have no difficulty showing you at least 50 set of before and after pictures for commonly performed procedures.
Being shown a handful of preselected images representing the best results of a providers career on patients who are excellent candidate for the outcome does not tell you what you can or cannot expect from any particular procedure.
Liposuction and fat transfer results vary greatly depending on who does the surgery.
These procedures have more outcome variation based on provider selection than any other plastic surgery procedure I’ve ever seen.
Bing board-certified in plastic surgery with years of experience and an overall good reputation does not mean somebody is good at either Liposuction or fat transfer.
Patients need to be vigilant in regards to selecting providers carefully. I suggest you do that I have a multiple in person consultations and take careful notes during each consultation.
When in doubt slow down and schedule more consultations.
I wish I could be more optimistic about treatment options for you but I don’t see any clear simple one procedure that’s going to give you a home run results.
