Mid-30s, healthy weight, have always had this 45 degree angle between neck and chin. There’s fat in there for sure, not just loose skin. I’m wondering if liposuction + skin tightening would produce a good result, or should I go for a full on Platysmaplasty/ deep plane neck lift?
Answer: Conservative treatment first. An in person examination would help to determine the thickness of the fat in your neck as well as laxity of the platysma muscle. I would always favor doing a conservative treatment first. A carefully performed liposuction of your neck should improve your neck contour and define your jawline This is minimally invasive, would have a relatively short recovery period and should give you improvement in your neck contour. In your mid-30's, additional skin tightening would be of marginal benefit but there are minimally invasive options to consider. If you feel like you've always had this, it is posible that your hyoid bone is just in a unfavorable location, which means that even with aggressive surgery, it can only improve so much. This is something that can only really best be determined by feeling in person. Based on your photos, I don't think lipo/skin tightening will do enough for you - at the very least you need some deep neck contouring to try to maximize what you can get with your hyoid position.
