The bulging appearance is because you still have the deep plane elements which cause the volume: submandibular glands, Subplatisma fat, digastic muscle bulging. Subcutaneous fat is not the problem, for that reason, liposuction didn’t help.
You will need Secondary Deep plane neck lift, treatment of the submandibular glands, digástric muscles and sub platisma fat, plus remove excess of skin through incisions behind the ears as well as the incision under the chin.

Thank you for your question and sharing your photographs. The impression from assessment of the neck contour is that it was not a deep plane necklift in your situation.

The configuration of the areas of fullness suggests most likely the prominence of the digastric muscles. Towards the lateral segment it most likely represents the prominence of the submandibular glands.

To clarify what is the underlying problem it is an option to use ultrasound scan to check what is the composition of the areas of fullness.

If there is significant fatty tissue still there, potentially liposuction can help. More likely however this would be corrected by a deep plane approach to the neck contouring.

Since the neck skin was tightened laterally, it may not be necessary to re-tighten and to address this problem only through an approach under the chin.

It would be worth discussing this with your surgeon in terms of the approach used for the surgery and the option of investigating the composition of the residual swelling.
