Yes, I think your goals are realistic with breast
Lift (lollipop scar) and fat transfer. Your breast size Woukd be slightly bigger . Some liposuction around the arm pit and bra strap area would also help in shaping.

The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers can be performed.

tuberous breasts are not simply small or underdeveloped breasts. The appearance of the breast can range from mild to severe, and typical traits include: enlarged, puffy areola, rather wide spacing between the breasts, very little breast tissue, sagging, higher than normal inferior breast fold, and narrow base at the chest wall.The type of surgery best for you will depend on the severity of your condition.My preferred technique for tuberous breast correction involves a multistep procedure where the breast tissue is released in a radial pattern to prevent herniation into the areola and placement of a custom implant size is in the breast to allow appropriate expansion of the breast in the restricted dimensions. This can often be done in a single procedure but sometimes may require two surgeries.That said, it is best that you visit a board-certified plastic surgeon for an in person examination. Only after a thorough examination you will get more information and recommendations.

Fat transfer to the breasts is a bit of a hit or miss procedure. Implants would work far better. If silicone worries you, go with saline. The fat transfer might give you what you want but the results are so unpredictable that I would steer you toward implants

Fat grafting alone will increase the volume of your breasts ,howeve in your case a breast lift may also be necessary

Do the lift first. If then you want more volume, add fat as a second procedure. Fat adds volume, but is less effective on reshaping the breast.

Thanks for posted excellent photos, it really helps. I recommend full or L=shaped lift with lipo 360 and fat grafting to lower poles. Fees in the $10,000 ranges. BTW your 'wish pic though possibly achievable no surgeon can EVER guarantee results.....

It sounds like maybe just a breast lift would be a good procedure for you. If you don’t need more forward projection then just doing a lift should get you pretty much where you need to be.

If you want to add volume then fat transfer can do that. Personally I don’t do a fat transfer at the same time as a breast lift but there may be surgeons who do them as combined procedures.

If you want forward projection then implants do that best.

In person consultations for the local board-certified plastic surgeons will help you get a more accurate assessment and better understanding of treatment options. Not all plastic surgeons do or have experience with breast fat transfer so if that’s specifically something you’re looking for then look for providers who specialize in that procedure.
