5’5” 120lbs 38 yrs, 32a/aa now 32b Is fat transfer a good option for going about 1 cup size larger ?

Fat transfer to the breasts can be an effective form of breast augmentation for up to one cup size increase. 200 cc per breast is typically the upper limit of fat injection recommended by most plastic surgeons. It is not that more cannot be injected; but on average, this has been determined to be the most that can be expected to live during one transfer. Multiple transfers would be required for additional cc's. In general, the abdomen and thighs are preferred donor sites but fat from other areas can be used.

fat transfer breast augmentation is a viable alternative for women who desire larger breasts without the use of implants. Fat transfer is also widely used in reconstruction of the breast after a mastectomy or lumpectomy. It can also be very effective in correcting the size and shape of the breasts due to genetic problems such as tuberous breast or Poland Syndrome, two conditions that look like misshapen or underdeveloped breasts.In addition, the following scenarios are also reason to consider breast fat grafting:• If you have chest areas that appear creased and sunken• If you desire more permanent correction than is provided by temporary fillers• If you wish to improve your body contour, revise scars, fill bodily depressions and rejuvenate the natural curve of your breasts• To fill in contour irregularities or hide obvious signs of breast implantsYou should discuss your concerns with a board certified plastic surgeon and make sure your surgeon understands your concerns and your breast augmentation goals. If you have donor fat, you may achieve about one half cup increase with the first transfer.  Once your breast is larger, then more fat can be placed at a second time.  You will not be able to achieve 275 cc's volume with the first transfer. Implants may be more realistic for you.

Compared to increasing implant size fat transfer is a lesser option.

A lot of plastic surgeons are apprehensive to do fat transfer and someone who has implants in place. The procedure puts the patient at risk for rupturing the implants during the procedure.  to avoid this my approach has been to remove the implants during the fat transfer part of the procedure and then either replacing back the same implants or a new pair at the end of the procedure. it’s all done as part of the same procedure and while the actual grafting is being done I place a temporary breast implant in the pocket called a breast implant Sizer.   Should we damage the size or than that does not have a significant consequence.   Grafting the tissues without an implant holding the skin under tension would make grafting technically difficult and I would not expect a significant improvement.

It is highly unlikely you’ll get one breast increase in cup size from fat transfer. If you want bigger breasts you should opt for larger implants.
