From the photograph alone in your description it sounds as though you're an excellent candidate for liposuction of the chin. Problem areas that don't respond to weight loss generally do well with liposuction. Liposuction of the chin can be done with local anesthesia only.

I treat this everyday in my practice. Properly performed energy based liposuction and physical fat removal will do fantastic to remove the fat, lift the skin, and contour the jawline. I pretreat everyone with Botox 1-2 weeks before surgery to relax the neck muscles, allowing for better retraction of the skin after surgery. Then after treatment we do skin tightening with Venus Legacy to improve the results and decrease the neck swelling.  Based on your photo alone, I believe a very conservative spot liposuction to your  jaw line  and jowl area should create a good improvement.  Your plastic surgeon should check for  how full your salivary glands are as well, since this cannot be improved with liposuction.  Very tight neck muscles called the platysma can create a  full appearing  chin as well . This is most easily treated  by botox.  So before you commit to liposuction, make sure  a board certified plastic surgeon evaluates you for these  anatomical considerations. Prashant Soni M.D.T

You are young and your skin should be sufficiently elastic to contract after liposuction giving you a well defined jawline. Chin advancement by using a chin implant or and osseous advancement genioplasty is beneficial if your chin is retrusive.. From your photos, you would appear to be an excellent candidate for fine cannula liposuction to give you a more defined jawline with a better definition between your cheek and your neck. I have had several young patients with the same condition, and the results can be striking under many circumstances. No neck lift is required. You do need to select a surgeon with experience involving liposuction in this region, because your fatty tissue is close to a nerve that moves your lower lip. While it is unusual, it is possible to have some asymmetry in motion for a period of time after the procedure. Good luck. You are on the right track.

This can be approached by altering the neck volume below the jawline or by enhancing the definition of the jawline by adding volume. I would suggest that you consider filler injection along your jawline to enhance its profile as a first step. The beauty of this as an initial approach is that some fillers, such as Juvederm Voluma, will yield a good and durable correction yet are still reversible if you don;t like the result. As a "test", Voluma can be injected and then a more permanent solution can be considered later.Good luck.
