From your pictures, you appear to be a very good candidate for what is called a tip rhinoplasty. Your nose is quite beautiful except for the distortion at the tip. It is not clear from photos, of course, exactly which cartilages need reshaping, but you can leave the bone alone. Good luck.

You are quite correct that a graft such as Alloderm or hard palate can be required in complex situations where the lower lid is scarred and shortened after surgery and where the lower lid can not easily be lifted with your finger. Sometimes skin grafting can be required. Usually, however, mild lower lid retraction is due to tissue relaxation rather than tissue shortening, and the condition is very responsive to tightening the lower eyelid with a canthopexy. To help determine whether you are a candidate for a canthopexy, try this test. Feel with your finger where the outer corner of your eye is. Now feel next to that the inner part of the bone of the eye socket just to the side of the corner of your eye. Ideally the corner of your eye is connected to the bone. If you can feel a distance between the corner of the eye and the bone, you are probably a good candidate to have a suture placed to reattach the structures together. That is what is called a "canthopexy" in medical terms, shortening the stretched out ligament that holds the lower eyelid to the bone. Speak with your surgeon--or another surgeon if you wish--about having your problem corrected. It will hopefully be very straightforward.

Hello Doctors, I had external lower Bleph. done 8 months ago, my eyelids are retracted - (Mild) . I am not looking for any type of agressive solution to my problem such as face lift or implants as I am only 32y old. Reading some answer on here I have realised that grafts are very much used to fix any type of eyelid retraction. In which cases alloderm and hard palate are used, the down side of them? How people feel having this grafts, any discomfort?

I believe that you would be a very good candidate for an injectable deep facial filler such as Voluma. It would give you some cheek bone fullness immediately and is also reversible if you do not like the effect. An experienced dermatologist is certainly qualified to perform this procedure. Sculptra is also a possible filler that would be effective if properly done, but it does take several injections over time and is not reversible. Good luck.

I live in an area were we only have certain doctors like dermatologists, hospitals, and so on. I want my face to be more feminine than masculine since I am transgender. Cheek augmentations I have been told is a lot safer than cheek implants. Can a dermatologist do this for me? The reason I ask is because I really do not want to have to drive 68 miles to get cheek augmentations done at a plastic surgeon.

While there are some instances in which cheeks can really be too full, I would recommend caution regarding any fat reduction in the cheeks at a young age. One of the biggest problems we have with facial aging is the loss of cheek volume and the appearance of hollowing. Reducing cheek fat in borderline circumstances can result in early facial aging later on. Stick with their being cute, and hold off on any procedure-at least at this time.

My cheeks are extremely fat and chipmunk like. Some days i think they're cute, and other days i loathe them. Is there any way to slim them down a little? Not anything as drastic as buccal fat removal, but something subtle. I have briefly tried facial excersizes, but they dont really work do they? Thank you doctors in advance :)

I must agree with Dr. Ishoo that you need to have your own personal consultation with some qualified plastic surgeons so that you can make your own decision. You have a lifetime of experience in making choices based on your 
intuition from personal interactions, and this is your most reliable 
source of information.&nbsp; Ask to see pictures, do not be fixed on laser vs. skin excision, and ask about whether you need canthopexies or not. Good luck on you investigation.

I am going to get a lower blepharoplasty at the end of the summer. Most likely it will be a transconjunctival lower bleph with laser skin resurfacing. I am also considering laser neck tightening but my main priority right now are the bags under my eyes. Really looking for someone who does this specifically. I've never had any work done and am finding it very hard to find the right doc because, of course, this is something people just don't discuss openly!

From your photos, you would appear to be an excellent candidate for fine cannula liposuction to give you a more defined jawline with a better definition between your cheek and your neck. I have had several young patients with the same condition, and the results can be striking under many circumstances. No neck lift is required. You do need to select a surgeon with experience involving liposuction in this region, because your fatty tissue is close to a nerve that moves your lower lip. While it is unusual, it is possible to have some asymmetry in motion for a period of time after the procedure. Good luck. You are on the right track.

I have a quite small face; so whenever I put on some weight my face get affected and I get the double chin. Even when I lose weight which I really hate my jawline is still not defined. I am only 23 years old and my height is 156 cm and my weight is 48 - 49 kgs. Losing weight is not an option for me because I will look too skinny if I do lost weight - so, I want a solution to this problem. Is it liposuction only? Or as well as necklift? What is the case here?

Those are good questions. A highly cohesive gel implant means that an implant rupture is less likely to spread out in your tissues than a rupture with a soft gel implant. That, at least, is the concept. I am not aware that this has actually been tested in practice. The price that you pay for this is that the form stable implants are firmer and not as soft as regular gel implants. How much firmer depends on the brand. There are other companies-most notably Natrelle from Allergan-that make form stable ("gummy bear") implants, and Sientra makes one of the softer ones. The primary characteristic of form stable implants is that they have a teardrop shape and are supposed to give a better and more natural shape. This is absolutely true in cases of postmastectomy breast reconstruction in which there is very little tissue cover over the implant. In implant breast reconstruction in general, the shape of the implant is the shape of the reconstructed breast. I am going to go a little bit out on a limb here regarding the advantage of teardrop shaped implants in cosmetic breast augmentation. These not the first teardrop implants. The original Cronin breast implants were teardrop shaped, and so were the much later Tebbets teardrop shaped saline implants from McGhan. Unfortunately there was a study evaluating what improvement in shape-if any-came from using those teardrop shaped textured saline implants compared with round implants. Photos of patients who had been augmented with each type of implant were shown to experienced plastic surgeons to judge whether they could tell the difference in quality of results. The conclusion of this study was, interestingly enough, that the surgeons could not tell which patients had which type of implants. This was confirmed with another study in 2006 with the present cohesive gel implants. My personal feeling at this point is that the teardrop shaped implants may give a better shape in flatter breasts, in ptotic breasts that need extra fill in the lower pole, and in certain other challenges in breast shape, but that the tradeoff in softness may not be worth it in many cases. Good luck in your investigation. When well done, breast augmentation can be a life changing procedure.

Thank you for the good pictures. You do have some tear trough thinning compared to a few years ago. While your partner evidently loves you the way you are, it would be nice to look especially good in your wedding photos. While I would not recommend any permanent treatment at this point, you can likely achieve some good improvement in the tear trough area with some gentle use of deep filler. This is a critical area because of the thin skin and tissue cover, but a hyaluronic acid filler such as Juvederm or Restylane has the significant advantage of being reversible if you do not like the effect. You do need to select a physician who is skilled at the use of fillers in this area. Some of the unused filler can be used for the nasolabial fold area as well. Good luck and best wishes.

I have always had sunken eyes with fuller cheeks but with a tired look now I lost my cheak counter since I turned 27 without loosing weight. Now tear through have become worse with nasolabial fold on left cheek. I have changed dramatically in a year .please tell me permanent solution .fat transfer or cheek lift or both . I have my wedding 5 months later .

It is difficult to answer your question without pictures, but here is some general advice. 1. Try to time any surgery after your weight loss has stabilized. Losing more weight after your breast surgery could easily cause a recurrence of your problem. 2. Is your problem sagging of your breasts or loss of volume or both? If part of the problem is loss of volume, a breast augmentation would certainly be reasonable. There are several different approaches that are excellent, and there are different types of implants that have different advantages and disadvantages. A small amount of sagging can sometimes be corrected with just an implant, but if you seriously fail the "pencil test," you will need a breast lift (or mastopexy). A breast lift or mastopexy can be performed by itself, or it can be combined with a breast augmentation at the same procedure, a so-called augmentation mastopexy. Congratulations on your weight loss. I would really advise your seeing a Board certified plastic surgeon to review your options personally.

Due to rapid weight loss i feel my breast lost all oomph firm and fullness. They have never returned to their normal state as i remember them.

Congratulations on your weight loss. This can be life changing. You are essentially doing your own liposuction. (Dieting has been humorously referred to as the poor person's liposuction.) After your weight loss is completed and has stabilized, you would undoubtedly benefit from some tissue tightening to achieve a trim figure to match you new smaller size, particularly after three pregnancies. That tightening would be from a tummy tuck. It is very likely that some liposuction might be performed at the same time as the tummy tuck to reshape your waist and your stomach contour, but the surgical procedure will primarily be a tummy tuck. Best of good fortune, and--again--congratulations.

I'm 5'8'' and currently weigh 197 pounds. In the last two months I have lost 28 pounds on a 1,000 calorie diet followed by an Endocrinologist and a Health Nutritionist. My goal is to reach 175 pounds and then work towards 155. Once I get to my goal weight which procedure would be better for me to have a Tummy Tuck or Liposuction? I'm 36 and have had three children.
