Whether performing an open or closed rhinoplasty, the soft tissue (skin, fat, and muscle) are separated from the bone and cartilage of the nose. It is the cartilage and bone that is manipulated to change the shape and improve the function of the nose. The soft tissue will then conform around the resulting bone and cartilage construct.

There is also a muscle in the upper lip that pulls the tip of the nose down. This muscle can be divided, in the right patient, to prevent the tip of the nose from dropping when a patient smiles. 

If needed, the nostril shape and size can be changed as well. This is often performed at the very end of the procedure.

addressed on a case-by-case basis. Age is indeed an important consideration, but it is not the
only consideration that a surgeon must take into account. I cannot assess whether you are a
candidate for a breast lift without a consultation, but in general, a 65-year-old could be a
mastopexy candidate if their aesthetic concerns are treatable and they are in suitable physical
and mental health. You may wish to consult with another board-certified plastic surgeon for a
second opinion. After a thorough assessment, they will be able to advise you on the most

Hello, thank you for your question. Total en bloc capsulectomy involves removing the breast
implant and the capsule that surrounds it as a single unit. It may also include repair of the
pectoralis muscle if the initial breast augmentation or reconstruction procedure involved
separation of the muscle. This decision is made on a case-by-case basis. Without photographs,
a physical examination or more details about your original procedure, it is difficult to determine
how your needs can be most ideally met. I suggest consulting with a board-certified plastic
surgeon who has experience with restoration of normal breast anatomy after explantation. Best

Hello and thank you for your question. A breast lift or breast augmentation will not directly affect
the structure or appearance of your sternum. However, breast implants or a combined lift and
augmentation may disguise your sternum and improve the overall look of your chest. As the
upper pole of your breasts lacks fullness, you may benefit from a high projection implant to fill
out that area and minimize the prominence of your sternum. I recommend scheduling a

Congratulations on your weight loss and thank you for your question! Facial volume changes
can dramatically alter the way a person looks and the skin does not always accommodate the
changes as completely as one would like. As some others have suggested, depending on how
recently your weight loss occurred, it’s possible that you are just not used to seeing your new
look and need time to adjust. Facelift surgery is generally not considered for patients of your
age, as it is designed to address significant wrinkles and skin laxity that young patients do not
have. Nonsurgical routes should most likely be explored before pursuing a surgical approach.
