thank you for posting your question. I some cases sublte changes are suffcient to correct a droopy nasal tip when smiling and a subtle dorsal hump. So-called preservation rhinoplasties may give you the results your are aiming for, without undergoing the more complex rhinoplasty procedures and the lengthier recovery. A preservation rhinoplasty maintains as many anatomical features as possible and changes the ones that are necessary without compromising the outcome (short- and longterm). To see if this surgical approach would be right option for you, a consultation with a rhinoplasty surgeon would be recommended. A  good understanding on both sides, about wishes, outcomes and realistic expectations, is really key.

To maintain balance and harmony in your nasal proportions, I would recommend a slight rotation of the nasal tip as part of the procedure. This adjustment can help create a more refined and aesthetically pleasing result.

Additionally, to ensure the nasal tip remains stable during facial movements such as smiling, I would suggest incorporating cartilage grafts for added support. This approach not only enhances the appearance but also provides durability and stability over time.

A rhinoplasty can rotate your tip and remove the dorsal hump for a more ski-slope tip.  Computer imaging can be used to help you visualize the changes and determine what is best for you.

A closed rhinoplasty approach can accomplish shaving down the dorsal hump, reducing your hanging columella, and refinement of the nasal tip with all of the incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. Regarding the dynamic droopy tip when smiling, it's very important to release the depressor septi ligament at the time of the surgery.

Thank you for your question. You are an excellent candidate for rhinoplasty, I think it is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. A rhinoplasty can be done with septal extension to prevent the drooping of the tip while smiling.  At the same time, the dorsal hump can be resolved.
