I was suffering from severe sleep apnea and had no clue I even had it. Dr. Sultan diagnosed the issue and treatment, which required major surgery. I was very scared and even cancelled previously scheduled surgery. This was no simple surgery, as it involved several procedures at once. Worst mistake I made was to start watching YouTube videos, not because they give you wrong info, but because I realized how sick I was and how serious those several procedures were. Met with Dr Sultan and rescheduled the surgery. I must admit that recovery was very hard, difficult and painful. Four months after the surgery, I now have the following benefits: Sleep Apnea is gone. Loud horrible snoring is nonexistent. Daily splitting headaches are no more. I can BREATHEEEEE!!!! I am not tired anymore, wake up rested and in good spirits. Sleep quality is very good. Blood Glucose is completely under control, for this i have also made changes to my diet and exercise. Lost 27 lbs. after surgery during recovery and with a total of 40lbs after diet changes and exercise. I highly recommend Dr Sultan. Don’t be scared, just communicate well with him and you will be good. On a last note, my son also had major surgery with Dr. Sultan, not sleep apnea related, and all turned out perfectly fine. Dr. Sultan did my upper jaw revision surgery after a different surgeon's operation left me with an upper jaw slant among other issues. I had reservations yet Dr. Sultan answered the plethora of questions and made me feel reassured of his capabilities and wisdom. I am currently a little over 2 weeks post-op and my orthodontist, my family and I are all happy with outcome. Not only did he fix the slant, he fixed the huge space between tooth #5 and tooth #6. I thank Dr. Sultan and his staff for taking on such a complex case and regret that I did not choose Dr. Sultan as my original surgeon to correct my extreme open bite. I included a picture and parted my bite to show the current bite alignment more. I highly recommend this experienced surgeon. Updated on 29 Apr 2019: My upper lip is still pretty numb and still not able to flare my right nostril. I have more movement of my upper lip on the left than on the right and the right feels stiffer and sore. I have checked incision area and seems to be healing...no discoloration. I also sometimes have a metallic taste in my mouth. I will bring it up to my surgeon on my next visit. I will post another update in a few more weeks. Updated on 2 May 2019: Week 5 Update of my recovery. I am experiencing upper lip numbness still...feels like pins and needles at times and my upper lip seems to move to one side. Can only still flare my left nostril and the cheek next to my right nostril moves slightly only when I try to flare right nostril. I hope is not nerve damage but will bring it up to surgeon. Updated on 2 May 2019: I also notice my lip moves to one side when I move them...talking, etc.... It is more noticeable if I purse my lips. I included a video showing what I am describing. Has anyone had something similar occur? Updated on 31 May 2019: My lip movement is better...I think the right side was more numb which affected lip movement initially. I see definite improvement and my orthodontist has me wearing bands now to finalize my bite alignment. I still have a taut feeling where the incision line is and included a picture of what my surgeon informed me is just scar tissue. Updated on 7 Jun 2019: I just realized the picture did not go through. There is apparently scar tissue and it still feels taut in this area. I took this picture today which is 9.5 weeks post-op. Other than that, I have had hardly any major side effects like I had with the 1st surgery. Updated on 25 Oct 2019: 7 months post op outcome of 2nd surgery- Lefort 1- 1 piece. I have not had any of complications or issues that I had after the first surgery. There is improvements in bite and jaw. There is still a slant although not as pronounced but I am not sure if that can be corrected without doing another 3-piece Lefort. I included a video to show my jaw movement, bite alignment and overall result. Updated on 11 Jan 2020: It has been nearly 10 months since my 2nd surgery with a 2nd surgeon/Sultan which did improve the major slant that resulted from the 1st jaw surgery in 2017 by 1st surgeon/Matouk (see my other review)...facial asymmetry in upper jaw area still obviously disproportioned. The way the jaw was set during 3-piece lefort in 2017 by the 1st surgeon/Matouk was not correct so even thought 2nd surgeon/Sultan tried to rotate it to a more aligned position with a 1-piece lefort in 2019, the middle part of upper jaw was still slanted not set properly. I rarely smile showing teeth due to this outcome which was initially to fix a class III open bite & included facial pictures of my results. At this point, I am learning to live with the jaw function & face I have now as a result. I no longer regret the surgery but would highly advise someone who is looking to do it, to consider this as an outcome. Updated on 10 Dec 2020: I have been experiencing pain in my joints on both sides now. I am not sure if it is due to the way my jaw moves but I have to see the surgeon again. I also have several other issues like the hygienist informing me to see periodontist due to deep pocket where the fracture was made between teeth #5 & #6 by the previous surgeon. There is also a tingling sensation as well. I hadn't realized I was basically using advil every day to relieve pain & discomfort until I had to stop taking it because it upset my stomach. It has become increasingly really hard to cope and keep my hopes up with every issue that has arisen and I am praying for relief. My family knows all that I have went through and I am blessed to have their love and support. Updated on 15 Dec 2020: This picture better shows my current canted upper jaw with my lower jaw level. The one I posted 2 weeks post-op was when I was wearing bands still and really does not show cant since my mouth was hardly open. I am concerned now with long-term issues of this deformity outcome since the most recent issues. Knowing now what I do, I would have never had the 1st surgery. Many things can go wrong and you, the patient, are left to suffer or deal with them even when it is due to others inaccurate decision and/or actions. When I started this journey back In 2013, I did not think I would still be in braces, have complications still and learning to deal with another jaw deformity going into 2021. I interviewed 6 jaw surgeons, 8 orthodontist....and countless dentists...had laser gum surgery. I did the leg-work, I put my trust in them and paid them my hard-earned money. This does hurt but not more than the pain I have been experiencing lately by trying to train my lower jaw to fit Into the slanted upper jaw. I want to cry but I am too tired to. I have no time to feel pity for myself because my ailing elderly parents need me to be there for them. I have a handicap sister who needs constant supervision. For me, this has become regret, frustration and disappointment. I am sorry for venting but I am so over it now. I now cannot be under an air conditioner without feeling pressure on my face, keep hitting my teeth on the right because of the slant, have pain on sides of my face when my lower jaw is angled into the slant, have tingling sensation also on right mid-face area and my bite only connects on one side. It seems my current photos should be the before and not the after. Updated on 22 Dec 2020: It's been a week since orthodontist had me stop wearing bands but I am still experiencing intense pain on both sides of my face. I feel it in joint and jaw area on both sides of my head. When I first felt it months ago, the orthodontist instructed it may just be due to the movement but it seems to be worsening therefore advil did help get me through the days until started upsetting my stomach. Orthodontist advised me to take alieve twice a day. I made orthodontist aware I emailed surgeon and would see him again today but alieve does not work as good as advil did. With the current pandemic I am actually kind of scared to seek help with this but it has become intolerable to the point where it has started to affect my life in general...work and personal. Also, since we have been on 12hr shifts...it is very hard with constant talking, jaw movement and physical pain to focus in a job where there is little room for error. I have no choice but to seek help! Updated on 13 Jan 2021: Today I was fitted for a dental stent to alleviate pain and discomfort I have been experiencing. I started wearing it on the way home from the surgeon's office to get use to it since it is suppose to ease pressure on the joints/jaw. The previous surgeon's 3-piece lefort connected my jaw higher on the left side, slanted down to the right in the middle & lower on the right...and later the previous surgeon sanded down the bone that was protruding up into my nasal passage on the left in an in-office procedure as a result of it being connected higher and creating pressure in that area. My cheek also sits higher on left as a result. Would having a 2nd 3-piece lefort to raise the jaw on the right and unslant the middle to align the upper jaw with the left take pressure off my joints/jaw and also even out the features as well? Most importantly, at the moment, I am hoping the dental stent helps and will update as time progresses. The stent feels loose on the left side where it is higher/more open but the surgeon says that he will make adjustments to it next week. He says also to cease all orthodontic work for the moment so I will call my orthodontist office tomorrow. Updated on 25 Jan 2021: Okay I have been wearing the stent since 1/13/21. Before any major adjustments, it felt stable where my jaw doesn't slide. After today's adjustment, the stent fits behind my lower right teeth instead of on top of it and it is not as stable...feels like it is sliding a little. The height of my pain was when my lower jaw was being jutted into this slanted upper jaw was like a constant 10 and I hope that is not where I am headed....back to square one. Lately it is more tolerable even though my teeth only meet in 1 spot on the right this way, the pain is not as intense and constant...(pain ranges from 3-7) now. I called on my way home and emailed the surgeon's office assistant and included picture of how the dental stent fits. I was one of last appointments Updated on 25 Jan 2021: How my upper jaw looks when it is open, you can see it is still canted though not as severe as the previous surgeon's misaligned surgery. I don't understand how the current surgeon cannot see the jaw is not aligned and says it is/was 'fixed'. Especially when I only have contact in few spots when my lower jaw closes naturally and is not being trained to painfully be jutted to the left. I am beginning to give up hope of ever feeling my teeth contact on left again. I still have an open bite, now instead of being in the middle part of my jaw, it is now on the left side of my jaw. But I guess the extreme cant of the previous surgeon's lefort 3 is not as as extreme after this surgeon's lefort 1. With that being said, I would never have chosen surgery as an option to fix my class 3 open bite if I knew then what I have experienced now. Please opt to do TADs. Surgery is too risky and can result in permanent issues like mine. Take my outcome into consideration if you are looking into surgery to fix an open bite. Updated on 18 Feb 2021: Waiting for 2nd orthotic due to 1st being very angled after the 1st alteration by surgeon as shown in picture and one left side fitting inside my lower teeth rather than on top of my teeth. My jaw is not in as much pain not using orthotic or bands for past 3 weeks but my bite is still way off as you can see when bands are not guiding my lower jaw into the awkward upper jaw slant. I had been wearing bands for over a year so I doubt a few more months will magically have my jaw trained into this awkward slant. I will keep updating progress. Updated on 29 Mar 2021: Since my last post, the surgeon asked me not to post on here while I am his patient. Since then he consulted that I do need a 3rd revision surgery to fix the complexed misalignment of the upper jaw. The revision he consulted was for double jaw surgery w/some kind of nose alteration. I inquired with front desk that I would like to speak with someone who had this double jaw surgery w/nose alter that he recommended as I was leaving...and later emailed asking why. I advised that my goal when I started and that I had reservations about having any other parts of my face altered since I had 2 upper jaw surgeries and neither went as expected. I talked it over w/my sister and we listed some questions that we needed to know to proceed... Questions I have...specifically involving canted/slanted upper jaw: How many procedures of this type have you performed? Also, how many times has there has been a need for revision? Do you have before-and-after photos I can look at for this procedure? What are measurements of where my upper jaw is being shifted/altered/adjusted and/or moved to? (I know this cannot be answered at this time but would eventually like to know before making any decisions.) (Will upper jaw be a lefort-1 1-piece again and if so, how is it being altered or positioned? If the jaw segments are found to be unlevel during surgery, what course of actions are planned to be taken?) To what degree of slant/cant is probable post-surgery? What is the efficacy long-term sustainability of cadaver bone with your past patients? How are the holes already in upper jaw handled? How is scar tissue handled? What is the rate of nerve injury for your patients? What is the rate of infection for your patients? How will this affect the sustainabity of my teeth? Other Questions and concerns: 3. What partners, assistants and/or personnel are involved and/or will be present during these procedures and planning of my care. 4. Have you ever been the subject of a disciplinary action from a professional board? 5. Does your practice use the latest technologies in oral surgery? 6. What will be expected of me to get the best results? 7. Were you specifically trained in the field of plastic surgery? How many years of plastic surgery training have you had? 8. Options besides blood transfusion you use? As a preference, I like my lower jaw and nose and really just would like the upper jaw misalignment corrected. I really just want this to be the last surgery of my life preferrably." Then on March 15th I received this email: After much thought and deliberation, I have made the decision not to offer you any further surgical care. I do not feel that I will be able to provide treatment, considering your stated goals, criteria and objectives that will meet your expectations. As you have expressed specific directives for your corrective surgery, I suggest you seek another board certified oral and maxillofacial surgeon proficient in complex orthognathic procedures. Recommended sources for another provider: (Included my insurance company and the American Association of Oral and Maxillofacial Surgeons info) I will continue to provide emergency care only for the next 30 days in regards to your present condition. In the event that you have a life threatening emergency, please seek care at the closest hospital emergency room. Once you have chosen a new provider, we can forward all of your treatment records. Thank you very much for your understanding in this matter." After the 2 surgeries that I have had to add to my budget, I cannot afford a 3rd surgeon to "try" and fix this. I need a surgeon who specializes in botched surgery involving slants/cants. My jaw actually is slanted upward on the left and downward on the right as you can see in the picture. I am not sure how this is going to be fixed without another 3-piece lefort 1. Updated on 30 Mar 2021: Since the bands have been orienting my lower jaw into this awkward slant, I am updating all my pictures without any bands to show a more accurate depiction of the before and after across all my reviews. Updated on 30 Mar 2021: This is a more accurate depiction of where my jaw actually contacts. I removed all pictures with the bands and am only showing pictures in my reviews without bands since the bands have been orienting my lower jaw into this awkward 3cm-4cm slant. Updated on 30 Mar 2021: Here is the orthodic before any alterations and after the 1st alteration. I was not able to wear it after any alterations but noticed that the pain and discomforting was not as extreme when lower jaw was not being trained to be angled into this upper jaw slant w/bands. Updated on 8 Apr 2021: Recently, my family and I have been going through all my pictures before and after surgeries trying to figure out how I got to this point. It was discovered that my mid-line has shifted to the left since last surgery. There is a 3d plate over my jaw and it seems that after it was put in and with the alignment the surgeon did, my mid-line shifted and is off-center now. This surgeon I am neutral about because this is a complexed slant and his intentions were to fix or improve it. I do believe I would have had a better outcome had he been the original surgeon. At this time, it seems my situation requires a surgeon who specializes in fixing complexed surgery-related deformities of the jaw. Along with the other issues besides the joint/jaw pain & discomfort., my jaw functions and looks so foreign to me. As a result, I do not smile in public or take pictures that include smiling or showing my teeth. I find comfort in the fact that due to Covid, it is recommended to wear a mask. Even with the mouth closed, my face looks off-centered and I am contemplating wearing a mask until this is corrected. It does consume me at times that this was afflicted upon me and that it cannot be undone as I would never have chosen what I have now over my open-bite. I am thankful to have the loving support from my family who has been with me nearly every step of the way and keep me grounded yet this is not the easiest deformity to live with. Updated on 21 Apr 2021: I have been getting consultations and it seems that the 3-piece lefort 1 needs to be re-done. I've had one surgeon's assistant do something the other's hadn't which is similar to this. The fractured sections are angled. Also, they did not feel the need for my lower jaw or nose to be altered...however a 2nd surgeon did. It seems that different surgeons have different ways of fixing these issues but regardless of this, I want to know this time the measurements of how/where my jaw is being moved even though I know there were most likely be some degree of slant but from what I have researched, not as large as what I have now.
