Careful review of the photograph provided demonstrates an increased distance from the left eyelash line to the eyelid crease above. This is consistent with ptosis, a condition where the muscle and tendon unit that lifts the left eyelid is asymmetric compared to the right. An interesting study of your photographs over the years and even in childhood may provide a clue if this a symmetry has always been present, congenital ptosis. Minor ptosis cases often go unnoticed or tolerated during adolescence, but after facial aging can become more apparent during young and middle adulthood. Botox can be used safely in patients with numerous prior medical conditions. Hypothyroidism is not a contraindication for Botox use and can be used safely in the vast majority of patients. In general neuromuscular weakness conditions or spastic conditions such as synkinesis are more diagnostically challenging and nuanced use of Botox in those case is more limited. In general Botox is FDA approved for three indications, forehead, glabella (11 lines), lateral orbital (crows feet). Botox is typically not used to treat ptosis or "lazy eyelid" In general, ptosis is corrected with tightening the tendon and muscle complex (levator muscle advancement). This elevates the eyelid which in turn decreases the eye brow elevation that is compensating for the low eyelid position. In general, patients typically wait for the Botox effects to wear off and then reassess your natural eyelid position and eyebrow position and have a consultation regarding correction of eye lid ptosis. The optimal combination and sequencing of multiple facial procedures remains challenging. Factors that help determine proper sequencing is determining if the vector of pull or tightening works well together or are competing with each other. Upper eyelid surgery (skin removal), and face lift (skin removal with deep tissue tightening) are very complimentary. A lip lift removes skin above the lip at the base of the nose to shorten the distance and improve incisor show at rest and smiling. This vector of pull is not antagonistic to the face lift vector of pull but may add too much surgery for a reasonable outpatient recovery especially when considering securement of the breathing tube under general anesthesia.
