These were placed above the muscle due to her muscular frame to prevent animation deformity. We were unable to use shaped implants due to concern for rotation in this revision procedure.

Occasionally, I have to reposition the implant under the chest muscle, if it was placed over the muscle initially. 5. Covering the implant with a sheet of collagen to prevent future capsular contracture. Capsular contracture occurs when the scar tissue around the implant becomes hard and tightens around the implant. 6. I sometimes add a mesh specifically designed for the breast. This acts as an internal bra to hold the breasts up in a higher position and prolongs the longevity of the result. This material is very safe. It is absorbed by the body over time and is replaced with scar tissue which acts as the new internal bra. 7. Many patients also need a mastopexy or breast lift if the breasts are very asymmetric or saggy in appearance. The type of mastopexy depends on the amount of loose skin present. The options include a circumareolar mastopexy (donut lift), vertical mastopexy (lollipop lift) and full mastopexy (anchor scar lift). 8. Lastly, I will occasionally recommend fat grafting to the breasts to fill in areas that need it. Some patients choose to replace their volume with fat instead of an implant. Which steps or techniques are used depends on each patient's specific case. Some patients need only a few of the above steps and some patients require all of the above steps to achieve the best results. The plan is customized for each patient to deliver the best and most beautiful results possible.
