Should I get a slight curve in my nose in my revision rhino?

Question

I am getting my nose redone by the same doctor, this will be my third surgery with my surgeon. My Nose is too wide from the front view so will be getting that narrowed slightly, I have very thick skin so I don't think there is much else that can be done to the tip. I have added pictures of My birth nose, then my nose after my primary op, my nose now and the last picture is of my nose edited with a slight curve using a plastic surgery simulator. Any advice/Comments would be much appreciated. thank you

Your first rhinoplasty caused a saddle nose deformity, due to over resection of the dorsum.The second rhinoplasty, aimed to correct the aforementioned problem, produced an acceptable outcome, maybe not perfect (a little thick and with scattered irregularities) but a good case anyhow. The key question now is: how did your surgeon correct the saddle nose or sunken dorsum?There are many methods to accomplish this, some disastrous or problematic (alloplastic prosthesis or implants, fillers, etc), other excellent (cartilage grafts, fascia grafts, etc), other average or doubtful. Some of these methods (we don't know which one was used in your nose) do allow the achievement of your wished mock pic with minor retouches, others force to a full rebuild of the dorsum.That is why I find of paramount importance you could tell me which technique, with scientific precision, was used in your second rhinoplasty, and based on this I'd build the reconstructive strategy to achieve your otherwise realistic goal.You must find a real master in revision rhinoplasty, your case is among the most difficult ones a surgeon can meet.

Curvature of the nasal profile is a personal issue and some patients ask for a slight convexity, some ask for a slight concavity,  and some patients ask for a straight nasal profile. This is to be discussed with  your surgeon so the they can give you the appropriate nose that you were interested in obtaining. Computer imaging performed in the office setting would also help in the communication process between the patient and the surgeon.

From the first post-op picture it is clear that the nasal dorsum was over-resected.  The revision did not result in smooth dorsal contours.  At this point you should have a very frank discussion with your surgeon to discuss goals and expectations.  Improvement in the dorsal contour is definitely indicated.
