You will need 360 tummy tuck, you have lose skin in the front sides and back, in order to have a good result, you need to remove all the skin like a belt, and also I recommended lose more weight to have better results

Your abdomen is very protruding because you have very thick abdominal skin and also a lot of visceral fat, fat on your back and very saggy abdominal skin.
Your visceral fat will prevent you to have a nice and tight muscle repair, so you won’t end up with a flat abdomen.
So if the purpose of surgery is yo make you look wonderful with a flat abdomen, better defined waist, nice shape, and if your fat deposits will impede to achieve the goal, in my opinion you must lose weight first. Liposuction won’t make your abdomen flat.
Your BMI should be below 30 and while laying down your abdomen must be flat to have a successful muscle repair.

It looks like you would benefit from tummy tuck and muscle repair due to your lax muscles. I do not like to lipo fat from the abdomen at the time of the tummy tuck as it challenges healing. I would recommend you consider lipo of the waist area. You should see a board certified plastic surgeon to assess your safety for the procedure. Your medical conditions and weight will be taken into account. Based on this assessment your surgeon might offer you the option of doing back lipo, but this procedure adds more risk to the overall surgical experience. If you do not address the back fat, you will notice it more once the abdominal area is removed. However, it can be done at a later time if you wish. Or, perhaps the abdominal surgery will inspire you to loose weight.

A circumferantial liposuction will increase the success of the tummy tuck. (You have an abdominal hernia, that area must be avoided during liposuction, and must be repaired during surgery.)

Typically when patients undergo a tummy tuck with liposuction for contouring they maximize the opportunity and have liposuction wherever needed. From your photo it appears you may have better results if you first lost weight. This would also give your surgeon the ability to optimize your results.

Better photos with full anterior and posterior views would help immensely in accurately answering your question, Form the photo provided, I have issues with your abdominoplasty. Your abdomen is tremendously convex (outward curved) which signifies both visceral (internal fat) and loose abdominal muscles + - hernias.
Visceral fat is only treated with weight loss (diet and exercise). The hernia may be reduced during the abdominoplasty but only if the tissue your surgeon is suturing too is strong and the internal abdominal pressure is not too high (either which may cause a failure of the repair and return to a round outward appearance).
I would require you to do weight loss prior to taking your money for an abdominoplasty.

A good plastic surgeon will look at the torso and design a procedure that makes the entire torso look appropriate. this means if someone has extensive skin laxicity that continuous beyond the front of the abdomen the procedure should be extended so that there is not a strange transition from treated to untreated area. this can be done by extending the tummy tug, doing liposuction or a combination of both.
