It is impossible to give you good advice based on this photo. It appears that neck and lower face lift could be a good option for you. However, without a detailed clinical exam it is not possible to tell you whether full or lower face lift is better. It really doesn't matter how you call it and how long the scar is; by lifting and filling (volume replacement in certain areas of face) you need to bring balance and harmony to entire face.You should seek a consultation with an experienced board certified plastic surgeon and discuss your options. The picture suggests that you may have a few options. This includes a mini or lower facelift. However, you may also be a candidate for non surgical options such as Ultherapy.
In my practice, a lower facelift and mini lift are often synonymous. It appears that you would have the most benefit from work done in the neck and the jaw line regions and your mid and upper face appear to be fine. Different surgeons use different verbiage often to describe the same procedure and I think a lower face lift or mini lift which in my opinion is the same procedure would be of the most benefit. It is best to evaluate you in person and find out about your aesthetic goals, have an understanding of your wishes as far as healing and "down time", give you a sense of what the risks and benefits and longevity are for your various options, and the costs associated. From your photos alone you would potentially benefit from non-surgical radio frequency tightening, either external (Tripollar) or internal (ThermiRF). This can be potentially combined with fractional laser skin resurfacing. If you were interested in a more immediate result a neck lift can certainly be considered. Make sure you are consulting with a board certified plastic surgeon experienced with all aspects of dance and neck rejuvenation, and make sure that your own sense of aesthetics aligns with those of your surgeon based on their photo gallery. It sounds like you are a candidate for a Mini-Facelift. My favorite technique is the MACS short scar Mini-Facelift. Developed in Europe, it is the ideal approach for the younger patient, not ready for a full facelift, or patients who desire a less invasive procedure with a shorter recovery time. From what you describe and the photo, you would greatly benefit from a lower (cervical-facial) lift. Please consult with an experienced, expert facial plastic surgeon. Avoid gimmicks, and the "quick-fix" sales pitch. In the right hands a very natural result can be easily obtained. Do your homework, and feel comfortable with the surgeon and look at their results. Thank you for your question and images. I tend to define things relatively simply. The length of the scar relates to the amount of excess skin. As the amount of excess skin increases in the lower third of the face and then the neck the more likely the facelift incision will extend behind the ear. A mini lift is a great procedure but primarily designed for patients with minimal skin excess. This tends to be younger patients, late thirties to mid forties. Older patients and those with more skin excess require a longer incision to remove the skin otherwise you would be left with a bunch below the ear OR what some surgeons extend the incision further onto the face. In my opinion I would prefer a hidden incision behind the ear rather than a longer incision on the face. Given your images I would suggest a standard facelift incision would be most appropriate to give you the best results. I hope that helps.RegardsDr Guy Watts
