Dr. Scott Trimas, MD, FACS is a double board-certified facial plastic surgeon and founder of Beaches Facial Plastic & Nasal Surgery Center, which has locations in Jacksonville Beach and Fernandina Beach, Florida. Dr. Trimas has been treating patients with care, talent, and respect throughout the Beaches and Amelia Island communities and beyond since 1994. As a facial plastic surgeon, Dr. Trimas performs both cosmetic and reconstructive surgical procedures of the face, head, and neck, including rhinoplasty, facelifts, and blepharoplasty. I wanted to have carbon dioxide C02 laser resurfacing for so long. It always seemed to me like a one and done procedure. I finally decided that I was going to do it. But, I wanted the real deal. Many places advertise CO2 laser, but they aren’t the real CO2 laser. I wanted the fine lines, dark circles, sun spots and small scars gone….permanently. I did extensive research and booked a consultation with Dr. Scott Trimas at Beaches Facial Plastic and Nasal Surgery Center. Having been to other plastic surgery offices, stepping into Dr. Trimas’s office was a breath of fresh air. He’s located in a medical building directly across the street from a major hospital. No fancy marble statues or faux gold furnishings decorating his office. It looked like any medical doctor’s office. Dr. Trimas explained to me what the procedure involved and how I would look afterwards. I have very fair skin and he told me that fair skin is the best candidate for the CO2 laser. He explained that I would be under general anesthesia and that I would not be able to go outdoors for two weeks. My face would be very red and that the redness could last up to four months. He said that there would be some pain, but that they do prescribe pain medication. Before we finished the consultation, I said to the doctor “my lines and scars will be gone, right?” “This will be a one and done, right?” He said yes. I booked the procedure before leaving the office and was scheduled for approximately two months out. The day of my procedure I arrived two hours early as I was told to do. Nurse Jackie took good care of me. She gave me a warm blanket, put me in a comfortable chair, went through my medications, and gave me the pills I needed to take before the procedure began. She also gave me a very generous supply of Alastin skin care (which after having used, I will never use anything else). And, she explained my the post laser procedures. Another patient was having surgery before me and nurse Jackie came in often to check on me and make sure I was comfortable. The last thing I remember is meeting with Dr. Yang… a partner of Dr. Trimas, and meeting with the anesthesiologist, Dr. Torres. Then, Nurse Jackie walked me into the surgery center which is located in the doctor’s office. I remember waking up and feeling so great. I remember just feeling great and was excited that I had done what I had always wanted to do. My boyfriend drove me home and I was still feeling great the whole ride home. When I looked in the mirror, I looked beautiful. I had some thick Alastin goop all over my face, but my skin just looked beautiful. At that moment, I had no idea what I was in for over the next two weeks. The next morning I woke up and that is when the redness started. I didn’t look so beautiful anymore. Over the next few days I became more and more red and my face started to swell. By the fourth day, my face looked like a basketball. The first four days were the worst. The scrubbing every two hours was the most difficult and the most painful. I definitely took the pain medication thirty minutes prior to scrubbing and as needed every four hours thereafter. After the two week mark, most of the skin was scrubbed off. My new skin was very smooth. I had no lines, no marks and my little scars were gone. I was still extremely red but most of the swelling had gone down. It has now been almost two months post procedure. My skin is beautiful and I am so happy. I am still very red, but I’m able to use a mineral powder to cover the redness so that if I go out at night, nobody notices. I never really wore a lot of makeup but now I don’t have to wear any. I still put on mascara and a little powder, but that’s about it. There’s nothing to cover up. The procedure took 20 years off my face. I am beyond thrilled with the C02 carbon dioxide laser, and I am beyond thrilled with Dr. Trimas and the entire staff at Beaches Facial Plastic and Nasal surgery center. Thank you, Dr. Trimas, Dr. Yang, Dr. Torres, Nurse Jackie, Nurse Susie, Leah, Loida and all that were so helpful and kind. I am a new and better me and I am forever grateful.

Excellent! The procedure was explained in detail ...both positive and negative. Yes, day 3 and 4 of recovery were tough but I was well prepared by the staff. I am extremely satisfied with the results. I have more and more people coming up to me and telling me I look great. It shaved at least 10 years off!

Dr. Trimas is so incredibly talented! I went to him for a revision septo-rhinoplasty. I had a botched primary rhinoplasty 15+yrs ago & had broken my nose multiple times since (due to an unrelated health condition). Needless to say my nose was very crooked, bossa tip, deviated septum, collapsed nostril, etc etc- pretty much any & everything wrong with it. Dr. Trimas miraculously fixed it using an ear cartilage graft. I can finally breathe & I'm not shying away from pictures anymore!! Overall I highly recommend Dr. Trimas & his entire staff. Everyone was so kind. I’m 5mo post op but will post pics soon. So glad Dr. Trimas was recommended to me & he literally saved my face! *I did have a very bad skin reaction/severe swelling due to the tape/splint used which left me with lots of broken capillaries all over my nose but hoping it can be fixed with laser later on.
Dr Trimas and his staff are amazing. I went to see him because at 62 I was depressed with the way my neck and jowls had started to sag. My neck was so bad you couldn’t even see the definition of my chin anymore! After consulting with Dr Trimas we decided a mini facelift would be the best thing to tackle all the things I had concerns with at one time. The result has been amazing ! No more saggy jowls and I have a clearly defined chin again! My husband can’t stop staring and complimenting me! I not only look 10 years younger but I have my confidence back again. I have had 3 microdermabrasion treatments with PRP spaced about a month apart using the skin pen. I am seeing my fine lines slowly diminish and my skin has become remarkably soft and blemish free. Very pleased with the results. The first couple of days after the treatment I was red and slightly swollen but nothing that was super noticeable or painful in any way. I started to notice skin flaking off very lightly on the 3rd day but it only lasted a couple of days. For the next few weeks my skin became super soft and my complexion became increasingly better and almost blemish free. Cayla at Dr. Scott Trimus's office in Jacksonville Beach, FL did an amazing job with my procedure and I highly recommend it if you are looking to improve any fine lines, acne scars, or just want to improve your overall complexion and get rid of unwanted blemishes due to sun damage etc. I give this treatment a thumbs up and I am loving my results! Updated on 21 Dec 2021: My neck is a ton smoother and less wrinkly after doing all 3 treatments. This was a nice surprise because I didn’t expect the texture to become so much softer and less bumpy in such a short time. The first two pics are an hour or two after my 3rd treatment spaced a month apart and the last pic is a week later. Definitely saw improvements in just one week after and now it’s been a month and My blemishes are definitely looking better and my face has never been softer! I am so happy with my results. I am a 56 years old and had upper eyelid surgery and Renuvion for my neck and upper arms. I can put my makeup on now smoothly with out all that extra skin and my eyes look so much brighter and open. My neck no longer sags and I have definition to my jawline and upper arms. Dr. Trimas and staff are extremely kind and professional . I have had several procedures over the years and keep coming back! I highly recommend Dr. Trimas for all facial cosmetic procedures. I keep everything maintained with botox and filler . Doctor and PA (Rae Gulve) are excellent!
WoW! Just wow. Had Submental Plication in 2013 which failed. Dr Trimus said he'd clean up the old scar tissue and plicate from the laterals of the neck instead of the center. Also J Plasma for lower face and neck tightening. My BFF did hers with oral sedation and was fine. I decided to go with IV sedation, so I'd be awake but juiced up... if you know what I mean ;) First let me say... His staff and anesthetist were so bright & cheerful, I was put right at ease. His anesthetist acknowledged my iodine & adhesive allergies and assured me they'd not be used. I was prepped and surgery tarted on time. I was in & out in 2.5 hours. Dr. Trimus entered the surgery operatory and it all began. He was gentle, attentive and fastidious. Since I had prior trauma in the area it was a little crunchy sounding. But other that that-- it was a breeze. I was able to walk out and go on about my daily activities. Walked the dog, made dinner and Netflix'd. Had chronic edema from old fillers, years old that I suspected were the culprit. I went for facial MRI and confirmed my suspicions. Dr.Trimas was hood enough to review MRI and conservatively address old fillers with hyaluronidase. IMMEDIATELY I noted an improvement! I am 24 hours out and can't believe how much better it looks. I may have to go back for 2nd session depending on how it settles 2 weeks from now.

The horizontal line is known as a mental crease- this tells me you have an active mentalis muscle. Botox in the mentalis would help with the protrusion you complain of as well as the mental crease. Once Botox is on board, using HA fillers in the chin can give you a more heart shaped face and reduce the "butt chin"
Botox in the mentalis muscle will reduce risk of migration of chin filler if you opt to trial filler again. I would recommend Botox to the platysma or a "Nefertiti neck lift" to give your profile the appearance you are looking for.
Botox to slim the jaw is an excellent idea- I would also recommend seeing an expert injector to inject a small amount to open your eye and help with brow symmetry. I would consider midface and possible temple filler before doing undereye filler again. I would recommend PRP undereyes over filler as well. Consider chin shadow filler to soften lower face. If the bulkiness in the lower face is not muscle or not responding to Botox, jawline filler may help give a more contoured appearance.
Chin filler is often placed directly on the bone to minimize risk of injected into a blood vessel. This is a common area for vascular occlusions, so product placement sounds correct. One single injection point is odd- from the photos it looks like you may have needed a smoother product in your chin shadows and something firm for the projection of the chin- but you only received projection which may have accentuated the volume loss creating the lines on the sides of chin. Hyaluronidase (used to dissolve) can destroy some of your natural hyaluronic acid along with the target filler. This is also a highly mobile area, the mentalis muscle is very strong, and can often push the filler and cause migration depending on product choice. Chin filler is best paired with botox in the mentalis to prevent this. From here you have a few options- Botox in the mentalis and DAO muscles can help smooth the area but it is temporary and will not address the indention left behind. I would recommend allowing a few months for the area to heal/recover and then consulting with a board certified facial plastic surgeon that works with an aesthetic injector to consider filler or more permanent products in the area if the problem persists.
