10 months ago, I got 415 cc high profile mentor implants both sides. My natural breasts have fluctuated in size due to hormonal changes and I was missing the volume. My experience and recovery was seamless, but I wish I would have thought more about how to combat the sagging that has already occurred. I am in my late 20s and have no children. Should I wait to think about any revisions to lift until after children? Would I need a lift or is there something else that could help me achieve lift?
Answer: Questioning a lift 1. Your implants are rather large for your chest and have contributed to stretching out the skin below your nipple. 2. The look you desire is not realistic without a push up bra. If someone has very tight skin before surgery, that look can sometimes be achieved, but you said you were deflated before surgery. 3. Sometimes the capsule can be tightened to move the implant up and in, but if you look at where your nipple is when you do that, it’s clear you would need a lift to get the look you want. 4. That said, no lift stays as high as your hands can push to due gravity and natural tissue stretch over time. My advice is to wait until after you have kids to have a lift unless you’re truly miserable. 5. I know they’re not what you want, but they still look great!

Answer: Surgery Your implants are too large and heavy for your tissue. You would do much better with smaller implants. Since your tissue has been stretched, you may need mesh for support. However, if you plan to have children, wear good supporting bras for now. Wait until you have had all the children you want and then consider a revision.

I believe that your implants may have been too large and too aggressive of a profile; this would be more easy to determine with an in-person physical exam. In addition, high profile implants project far too much to produce a more natural breast shape. Essentially, high profile implants were created for breast cancer reconstruction; they have a large amount of projection for a very narrow given breast base width. I would recommend adding Galaflex (internal bra) and performing an "inferior mastopexy" to shorten the nipple-to-fold distance on both breasts. This will help provide long term support and set an appropriate distance for the breast.

Answer: Do I need a lift? If you are considering a breast lift i would suggest that you wait until you have had your children. Regarding other options, you may achieve some improvement by repositioning the implants. To predict the degree of improvement would require a physical examination.

Answer: Explant with Bellesoma Method Unfortunately, you have not provided before photographs. However, you now have large low breasts that will continue to descend down the chest wall. If you want to achieve higher perkier breasts then I recommend explantation with lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery.
