I had a breast lift 6 months ago with 415cc implants, but I am not happy, I feel that they're still very sagging and never liked how separate they're, when I lie down the implants go completely to the side ending with the flattened chest, before the surgery my doctor said it was going to be behind the muscle, but after the surgery she said it was like a dual plan, because she looks for natural results but I didn't want that, I want my cleavage rounds and high.
Answer: Explant with Bellesoma Method As you can see, the anchor lift did not significantly lift your breasts high enough. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.

Answer: Surgery Your lateral pockets can be tightened, but you may need mesh. You might have some excess skin and that can be excised at the same time. Do not go bigger for you will not support heavier implants. Long term, you would do better with smaller implants.

Answer: Breasts I would recommend tightening the implant capsule with sutures, placing an internal bra (Galaflex), and performing another breast lift. From the photos, it is clear that there is inadequate support for the implant in each respective pocket. In addition, there is too much excess skin/breast tissue. I would also recommend finding a surgeon certified by the American Board of Plastic Surgery. I would particularly recommend finding one with excellent experience in breast surgery.

Answer: Revision after an augmentation mastopexy Revision after an augmentation mastopexy Typically it is best to wait 9 months or more after the initial surgery before attempting enter revision. This allows the tissues to stretch and settle into place and allows the tissues to soften and allow for additional surgery and healing. In your photos it appears that you have a great deal of loose skin. The breast lift can be adjusted on both sides for overall better improvement. Possibly a different profile implant If this is not the first revision and previous treatments have failed. Then I will immediately move to adding the addition of a Surgical mesh to help close and reinforce the corrected pocket. This will reduce the internal space and help prevent recurrence of the displaced implant. Implant displacement can be in any direction. Drop-out is one version where the implant is too low. This is sometimes confused with breast stretch where the distance between the nipple or areola and the inframammary fold stretches and increases this distance too much creating a sagging appearance of the breast. A Waterfall deformity is another variation where the breast tissue slides off of the implant leaving the implant in place but the breast tissue hangs off and drops, giving the appearance of sagging. Displacement toward the cleavage can create asymmetry or if more severe, a “Uniboob”, where one or both implants are crossing the midline giving the appearance of one breast.

Answer: A revision could help Hey there! Excellent question - based on your photos, I'd say that there was insufficient support for the size of implants used, especially given the appearance of your tissues. This is typically a case that I would recommend an internal bra like Galaflex as well as a smaller implant, though in a higher profile. This would allow the implant to remain higher and more centered while still giving you a full volume appearance. A revision would look to achieve this and you may consider this at some point in the future. Hope that helps!
