Should I get Submentoplasty, neck lift, liposuction, or other? (photo)

You have skin laxity with visible relaxation around the mouth and jowls at rest as well as some aging changes in the neck.   The only procedure that will definitively give long term correction is a lower facelift that addresses the again changes around the mouth as well as the jowls and neck.   In my hands a minilift would improve the neck and jowls.   A variety of smaller procedures and even noninvasive skin tightening could be offered with much less effect.   I do all of my facial aesthetic procedures under local anesthesia and sedation which has advantages in recovery as well as being less stressful for your body.

Almost always, the first and most important information I try to understand at consultation is what final result is desired?  Your aversion to general anesthesia understood, but how much are you willing to compromise the quality of your result to avoid it?  
Responsible professionals must consider these and other factors before offering treatment recommendations, in my opinion.

You are an excellent candidate for a standard facelift. Don’t get confused by all the different terminology. It’s just word play. I personally don’t like fillers with the facelift and that includes fat injection. Just extra expense with no benefit at all. A submentalplatysmaplasty is part of most standard facelifts. I prefer this done under general anesthesia but it’s okay under local anesthesia with sedation as well.

 Much more information is needed, such as a full set of facial photographs from all angles to make a determination on how best to proceed with three different operations. In our practice, a necklift procedure is also known as a submental-plasty. This procedure involves removal of fat deposits above and below the platysma muscle and additional platysma-plasty. No skin removal is performed. Liposuction can only remove fatty deposits above the platysma muscle, while surgery is  performed to remove fatty deposits underneath the muscle. When there's loose and inelastic skin in the neck along with jowls, then a lower face and neck lift is required. Both of these procedures are performed under general  anesthesia by a board-certified physician anesthesiologist for patient safety and comfort.

None of the non-surgical treatments will be beneficial, nor will be a mini facelift. A well performed lower facelift (which includes the neck or submentoplasty as you called it) will be by far the best. I do them all under GA because it takes a few hours to do and it is easier on the patient.
