Small lump formed around breast incision line 4 years post op - could it be related to the surgery? (Photos)

Having this evaluated is always a very good idea to rule out any malignancy, the fact that it is on your incision line could mean that it is an old suture knot working its way out. If your surgeon used permanent sutures the knot would still be there and could in fact be working its way out over this time. Most of the time absorbable suture is used there.

Based on your post photos, what you are describing as a suture granuloma. That can be done by excision under local anesthesia. Short answer is yes it could be a granuloma or cyst. It could also be a cancer.
