I would recommend a full breast lift to maximally lift, shape and tighten your breasts. If you want more volume after the lift, then you could consider modest augmentation if desired.. Thank you.

fat transfer breast augmentation is a viable alternative for women who desire larger breasts without the use of implants. Fat transfer is also widely used in reconstruction of the breast after a mastectomy or lumpectomy. It can also be very effective in correcting the size and shape of the breasts due to genetic problems such as tuberous breast or Poland Syndrome, two conditions that look like misshapen or underdeveloped breasts.
In addition, the following scenarios are also reason to consider breast fat grafting:
• If you have chest areas that appear creased and sunken
• If you desire more permanent correction than is provided by temporary fillers
• If you wish to improve your body contour, revise scars, fill bodily depressions and rejuvenate the natural curve of your breasts
• To fill in contour irregularities or hide obvious signs of breast implants
You should discuss your concerns with a board certified plastic surgeon and make sure your surgeon understands your concerns and your breast augmentation goals. Only after a thorough examination you will get more information and recommendations.

Fat transfer is a relatively less reliable means to add volume to the breast when compared to the alternative (breast implants).

While it can be very helpful for correction of contour irregularities, the unpredictability of fat transfer makes it a suboptimal means of gaining size.

As always, discuss your concerns with a board certified plastic surgeon (ABPS).

At size 34 E, you have enough volume but your breasts are flat because of the hanging skin. You need a breast lift and I recommend The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained.

I would start with a breast lift only for now. Excess skin would be removed. You would keep all of your breast tissue but now it would be all in the correct location. Let it heal. As long as you have excess fat to give, consider fat transfer after full healing. We perform both of these procedures in our office under local anesthesia. It’s very safe, has a much better recovery, and the results are fantastic.

The principal of a breast lift is to remove some of the excess skin on the breast, but retain all of the breast tissue volume. When all of this volume is then tucked inside this smaller envelope of skin and sutured closed, it plumps out the remaining skin envelope so that the breast look fuller at the same time the breast is elevated. If your skin quality is poor, due to stretch marks, the option of using a support matrix placed under the skin to help hold the breast in its elevated position could be considered. These matrix incorporate into the tissues relatively rapidly and increase the strength of the skin repair to help maintain the position of the breast. I do not perform fat grafting as I feel that the results are not reliable nor predictable. The possibility of using some RF or microneedling to help tighten the skin envelope is a possibility that you could look into although I am unsure of the longevity of the results from this.
