Hello, please help me! I have photos attached. Over the last 14 months, I have lost 80 pounds. 37 years old. I already had some minor jowls prior to weight loss. Now that I have lost all the weight, they are even more noticeable. I am wondering if I would be a candidate for a mini facelift, or if fat transfer or filler would be better. Already tried all skin tightening treatments under the sun. Nothing has worked. Thank you in advance!

Yes , the area of laxity around the mouth and jowls Woukd be improved substantially with a lift procedure that appropriately lofted the midface. This can best be done by a deep plane approach, but with a shortened scar and more selective dissection. The results should last many years. For weight loss patients to it is entirely appropriate to consider surgery in a Katie t in their thirties.

While a mini facelift can effectively tighten the jawline, addressing the mid-face sagging with a mid-face lift would likely give you the best results. This combined approach would reduce sagging in the lower face and restore fullness in the mid-face, leading to a more natural and long-lasting outcome. Best of luck to you!

The biggest thing that I see is major volume loss in your lower face, and that's probably why you are noticing the "jowling". You almost look like you have no lower face, especially in the posterior jawline. I would recommend adding volume to these locations instead (although be prepared that you may need a fair amoubnt) - this can both tighten the skin and also bring the face into more proportion.

The distance between your nose and upper lip is too great. You would benefit from a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners.

Based on these limited photographs, you do not appear to be a candidate for any type of facelift procedure. You also appeared to have a recessive chin and prominent pre-jowls on both sides of your chin, therefore you'd be a candidate for placement of a small chin implant to augment your chin further forward and gain additional width to widen the area in the pre-jowl sulcus. This procedure can be performed under local anesthesia, which takes about 30 minutes in the office setting.

Facial aesthetics is a very complex topic.
A lot of fundamentals have to do with bone structure.
This is especially true for young people.
Issues related to the lower 1/3 of the face are often a reflection of the size and shape of the mandible, especially as it pertains to the maxilla.
The profile picture you’ll see that your upper lip has significant more forward projection than your lower lip.
The lip relationship is based on your dental occlusion, and based on this, you must likely have a significant overbite.
The overbite intern comes from the relationship between the mandible and the maxilla.
When the body of the mandible lacks sufficiently in forward projection, it doesn’t give a strong foundation to hold us off tissues of the lower, 1/3 of the face and neck, and then these individuals premature gels are going to form.
This often also is associated with loss of jawline definition and sometimes a premature double chin.
There is no effective skin tightening treatment, other than direct surgical excision, which is what a facelift is.
Proper jaw surgery is usually considered fairly undesirable by many people so they look at soft tissue options instead.
There’s nothing wrong with having a facelift though I’m not a big believer in many procedures.
Either do the procedure and do it correctly or don’t do it is the way I see things more commonly than not.You are most likely get a lot of different opinions for what would be the best treatment.
That is often the case when there is no single best treatment option.
That is also often the case when understanding the underlying primary problem is complex or difficult to understand.
Plastic surgeons tend to be more soft tissue oriented, and see solutions through that lens.
Oral surgeons on the other hand tend to be more bone oriented and will often see the problems based on skeletal imbalance.
Even if you opted for not having jaw surgery, it’s not a bad idea to get a formal consultation from an oral surgeon or a plastic surgeon with cranial facial training.
