In your 40s you do not need anything too aggressive. A mini lift would lift your cheeks, eliminate the lines around your mouth and tighten your neck. A lower face and neck lift is only going to give you minimal results to tighten up that small amount of excess skin in your neck. Probably best to just wait until you have more facial changes of aging before undergoing any type of a facelift procedure.

Based on your post of photos and your age, the option of a deep plain facelift for the higher risk of complications, longer healing time, and expense does not outweigh the use of a mini face neck lift with muscle supplication as far as faster healing time good results and less expense.
