Hello Doctors. I'm unhappy with the heaviness of my lower face and undefined jaw line. I'd like to get something done to address the excess skin, tighten and lift the lower face and neck and also make my jaw line look more defined, I'd like a more heart shaped face vs the round or square shape I have now. I am wondering what type of face lift is best for me (deep plane, mid, mini etc)? Won't a mini / lower lift not last as long and need to be repeated soon? I'm 40. Thank you.

Answer: Lower face and neck aging Thank you for providing photos. At this time it appears that you are much too early for surgical facelift. You do not have enough skin/soft tissue laxity to provide you a robust result. Ellacor is an FDA cleared procedure for the lower third of face that removes 5-8% of full thickness dermis per treatment; this results in a tighter lower face, less fine lines/rhytids/jowling. A great collagen boost also occurs with Ellacor. Other than that, doing laser/and or microneedling treatments will keep your dermis thick, boosted with collagen through the aging process.

Answer: A mini facelift would be your best choice. You have excellent cheekbones and facial shape, have kept yourself in very good shape and have a well-defined jawline with minimal jowls and a small amount of platysmal muscle banding in your neck. At 40 years old, your skin elasticity should be excellent. Therefore, I feel that your best option would be to have a mini facelift, with suture plication of the SMAS layer and your muscle to help smooth your neck contour and sharpen your jawline slightly and smooth the skin of your jawline and cheek.. I don’t believe anything more extensive is needed or would benefit you. Your excellent skin elasticity should give you many years of a solid improvement. Best wishes.
