Mesh or No Mesh to bring breasts closer together? (Photos)

Question

I have ~20 year old saline implants under the muscle. I have never liked how wide my breasts appear and never have cleavage. I am currently about. 34D. I’d like to get new implants and have seen several surgeons. Some say I need mesh to re-position my breasts closer together and some say mesh won’t work because of my particular breast footprint. . I’d like to go a little larger too.

At this time, I recommend explantation and breast lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Thank you for your question and photos. First of all, you will never have close cleavage due to your bony chest wall architecture. You have somewhat of a barrel chest deformity which makes your implants slide to the side. The implants sit on your chest wall! Also, you have notching of your rib cage medially, one side higher than the other which prevents your breasts and implants coming closer. Mesh might not help in the long run. If you were my patient, I would recommend a breast lift/augmentation. This will help bring your nipple/areolar complexes to be placed more medially. You would benefit from a low or moderate profile implant. See a board certified plastic surgeon experienced in all aspects of breast surgery for an in person consultation/evaluation. Good luck.
