The patterns you see is typical of bottoming out, and the rippling is common with older saline implants. Fixing this usually requires pocket repair to lift and support the implant back into the correct position.
In this situation, pocket repair is the correct approach. Simply switching implants without rebuilding the pocket would not lift the implant or restore the upper-breast shape you’re aiming for. A more cohesive silicone implant will also help to improve fullness and reduce rippling.

Hi! 41 y.o F, 130lbs. I have had Saline implants for 14 years! They are now starting to ripple and are really low with no upper pole fullness. Any advice as to what you see in this photo? What kind of implant would fix spacing, fullness etc. I am planning on Motiva Implants.

The raised, red scars and the lighter areas you see on the areola are consistent with hypertrophic scarring and partial pigment loss, which can still be seen at this stage. Some pigment may return over time, but it can take up to 18–24 months for the color to stabilize. If the lighter areas don’t improve, medical areola tattooing is a reliable option to match the color.

It's been a year and 2 months since this surgery. Scars are pretty red and now it seems like I'm losing pigment on areolas. Is this normal? Will they go back?

Hi, your question is much appreciated.
Tip refinement is done by reshaping the cartilage and reinforcing it so the nostrils stay supported and don’t collapse, even when you smile. And your nasal bridge and bulbous tip can be refined in the same rhinoplasty. The bridge can be narrowed, and the tip can be shaped to look slimmer and more defined.
Overall, in my opinion you appear to be a good candidate for both improvements together.

I have never liked my nose. I have a wide bridge and big wide tip of the nose. My nose seems to also get much larger when smiling. Is it possible to slim the nasal bridge bone and narrow the wide tip at the same time? How can the tip be slimmed and come to more of a point without the nostrils collapsing?

You look like a solid candidate for rhinoplasty. Your bridge doesn’t need much change, in my opinion the main opportunity is at the tip, where refinement can make the nose look more defined and balanced without altering your natural look.
Some surgeons can provide you the opportunity to show you digital simulations during a consult via devices such as Vectra. It’s not a definite promise of the final outcome, but it gives you and your husband a clear idea of the expected direction and helps make the decision easier.

I was wondering if I would be a good candidate for a rhinoplasty. I have always disliked the tip of my nose. Is there a way to see a before and after at a consult to see what the final result might look like. My husband is hesitant and I think it would support my decision more.

Indeed, septoplasty along with a conservative rhinoplasty seems suitable for you. Your airflow problem and the collapse of the nostril can be fixed at the same time along with minor cosmetic refinement. It is very clear from this axial slice that the septum is deviated and one nasal passage is significantly narrowed. The asymmetry indicates that the obstruction is a structural one. You also said that the nostril collapses when you are breathing, which might be the internal nasal valve weakness. These results correspond to functional breathing problems which are corrected by septoplasty and, if necessary, valve repair. 
The most proper type of surgeon to do this is a facial plastic surgeon or an ENT surgeon who regularly does functional rhinoplasty. Both are equally good as long as you pick someone who is skilled with both breathing correction and aesthetic work in the same operation. 
Regarding insurance, the majority of surgeons send the breathing-related findings (e.g. nasal obstruction, valve collapse, septal deviation) for insurance approval, and you would probably be allowed to pay only for the cosmetic portion. They don't cover cosmetic changes.

I would like have a septoplasty and minimal rhinoplasty combined. What type of surgeon should I see? Based on the CT, would insurance pay for this? I do have restricted airflow on the left. My nostril collapses when breathing solely from that side. Thanks in advance!

From the photos your bridge is looking already balanced, and the main concern is the bulbous, boxy tip. Tip refinement can narrow and define this area without over-rotating the tip or exposing the nostrils. You do look like a good candidate for a tip-focused rhinoplasty. 
A well-planned tip plasty should not make your nostrils more visible. The goal is controlled definition, not lifting the tip too much.

I've always wanted a rhinoplasty but I’ve never been sure what kind of results I should anticipate. Not happy about how boxy or bulbous my nasal tip. I have always been concerned that if I had a nose job it would make my nostril more exposed. would I be a good candidate for a tip plasty?

Greetings, thank you for your question as well as the attached photos. Yes, both of these procedures can be confirmed under one surgery. A rhinoplasty can address everything you mentioned: smoothing the hump, straightening the nose and refining the bulbous, square tip to create a smaller and more feminine shape. An upper blepharoplasty would help open the eyes and reduce the hooding you’re seeing.
Doing them together gives one recovery period and does not compromise results.

I am considering a rhinoplasty but have multiple concerns I’d want it to address: firstly, the hump on my nose, secondly the fact it bends to one side and lastly I feel I have quite a bulbous and square tip. Would a rhinoplasty be able to address all 3 issues, and result in a smaller, more feminine nose? I would also be interested in an upper Blepharoplasty to open up my eyes and reduce hooding - can these 2 procedures be combined?

I'm afraid yes, it's possible to lose volume in your buttocks during your weight loss process. The fat will act like any other fat in your body. The amount you lose will depend on how much weight you lose and how your body naturally stores fat.
Most people maintain some shape, but some volume loss is normal.

Most patients need to avoid glasses for about 6 weeks so the nasal bones can heal without pressure. If you can’t use contacts, you can try wearing very light glasses so there won't be a lot of pressure on your nasal bone.

You do have a visible dorsal hump, and your tip is both droopy and separated (bifid). A tip only procedure would not correct the hump, and lifting the tip without smoothing the bridge would actually make the hump look more pronounced.
To fix your main concerns, you would need a rhinoplasty that includes hump reduction plus tip refinement and rotation. This gives a smoother profile and corrects the droopy, bifid tip at the same time.
