I had breast augmentation surgery. Now I have asymmetry — the left breast is lower and the nipple points downward. My surgeon wants to do a revision with larger implants, but I’m not sure this will fix the asymmetry. What should be done to correct this?”

I understand your concern. However, without a proper assessment, it would be difficult to determine what is wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Thank you for your question.  I have been in practice for over 30 years and revisionary breast surgery is a large portion of my practice.  Your overall result is nice, however you could ask your primary surgeon to perform a doughnut lift on the right side.  Otherwise, I would consider elevating both breast mounds with an internal bra/capsulorrhaphy and performing a revision of both mastopexies as your nipples still point slightly down. It appears in addition to augmentation with implants you also had mastopexy, breast lift.  Correction of breast asymmetry depends on what is causing the visual asymmetry.  Differences between right and left may be related to capsular contracture, implant malposition, different breast volume, skin surface area, nipple position, inframammary crease position, or some combination of these and/or other factors.  Your surgeon will conduct this analysis, formulate a surgical plan, review with you what to expect, and answer your questions so you can decide how to proceed.

Without photographs from different angles and a physical examination, it isn't possible to recommend a specific correction. That said, several details are also important in making this decision: how long ago your surgery was, how much asymmetry your breasts had before surgery, how much tissue was removed, and how many cc the implant placed was.
In general, simply using a larger implant does not lift a low nipple or correct a downward-pointing one, so these details &mdash; along with an in-person evaluation by a plastic surgeon &mdash; are what will determine whether a lift, an implant adjustment, or a combination is the right approach for you.
