Too low and no prominent inframammary crease - Are my implants bottoming out? (Photos)

Question

I feel like my implants are too low. I do not have a prominent inframamary crease anymore. My nipples also seem to have the characteristic upward position that happens when implants bottom out. I have an appointment with my surgeon, but I want to get some other opinions while I wait for that.

It appears that you are starting to "bottom out". This can be due to many reasons, but usually it is some over release of the IMF/inferior border/sternal attachments of the pectoralis muscle and placing too large of an implant without adding some type of soft tissue support. I would recommend seeing an American Board of Plastic Surgery certified plastic surgeon, ideally one who also completed an accredited Aesthetic Society fellowship or is a member of the society.

bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery.

Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies.

If you are concerned, please check it with your plastic surgeon for reassurance and to further discuss your options and possibility of revision.

Your breasts appear asymmetrical with one nipple slightly higher than the other. The breasts are low and lateral. You will need the pockets redone higher and more medial, you will need plication inferiorly and laterally. You may also need a donut mastopexy to resposition the nipple-areola complexes.
