Can this facelift be improved? (Photo)

Hello, hope you are well. There is plenty of room for improvement with your surgical result. I would suggest a revision facelift (and not just skin excision "tuck up" which will cause hypertrophic scars and an unsatisfactory outcome). The proper way to do this is via a revision modified deep plane approach which provides access to the buccal fat. If it is needed, a conservative amount of buccal fat can be removed via this approach to sculpt the lower cheeks--I consider this to be just a component of deep plane facelifting, along with parotid reduction, etc. You may also consider a lip lift and fat grafting for complete facial rejuvenation. Did your lift actually lift? Sometimes you are better off going with a full facelift revision to lift the deeper tissues to achieve a full face rejuvenation. Most of my patients go with a full face and neck lift to achieve their goals. In most cases our goal is for a facelift to last 10-15 years if maintained properly with medical grade skin care and resurfacing lasers.

You still have jowls, which were not treated adequately by your deep plane facelift (this is a shortcoming of many deep plane techniques). The answer would be a revision facelift with techniques directed specifically at reducing the jowls.

A revision should help correct the residual cheek ptosis, marionette lines, and excess neck skin; however, probably not a a second deep plane lift but a SMAS plication lift would be safer.

I does not look like you need anything major, like face lift revision. Neck muscle bands can be treated with Botox, jowls with liposuction with small cannula, nasolabial folds and marionette line with fillers. You may also consider lip lift to improve appearance of perioral area? In person, second opinion consultation, may help. Hope this helps.

The easiest solution is fillers in cheeks and you will see a change. If you like that look and want something longer lasting then get fat grafting.
A revision facelift will certainly help but still will not address the loss of volume in the cheeks. So you have the options of
1. Cheek Fillers
2. Cheet Fat Grafting
3. Cheek Implants

I would not recommend buccal fat pad removal.

The main problem I see is that you still have prominent jowls. I agree that a Dr o dart lift would be of some benefit. The Jules can be liposuction or the buccal fat iAds removed. Both reasonable options

Based on your photographs you appear to have residual laxity in the facelift deep plane--specifically one can see three oblique down and out lines--- (1)lower lid muscle, (2)nasolabial and (3)marionette.
The easiest/best(in my opinion) is a non surgical rejuvenation of the surgical deep plane. With my procedure the facial soft tosses are elevated, tightened and rejuvenated---and yes at age 65
One week downtime

A full set of facial photographs from all angles including both side profiles and your three-quarter profiles are necessary to make a determination about how best to proceed.
