Thank you for your question regarding which procedure would get you the best results for your buttock, namely a Brazillian Butt Lift (BBL) or posterior body lift. Let's examine the BBL. For a successful BBL procedure, you have to have fat and the place that's accepting the fat, namely the buttock, has to have enough skin elasticity to accept the fact. In my opinion, you do not have much fat. Though you may feel that you have a significant amount of fat in your love handles and perhaps the abdomen, in my opinion, you do not have much fat. Though this fat can be transferred, I do not think the effort will be worth your buttock results. If you closely examine your buttock, you have excess hanging sagging skin in the lower portion of the buttock where the buttock meets the posterior thigh. Some patients call this a "diaper butt". To fix this, you will need a significant amount of volume added to the buttock. Again, you do not have much fat to transfer from other body parts. Therefore, what are your options? First, you could gain weight to create fat for fat transfer. In your case, I would suggest you gain 10-15 lbs which can then be transferred to the buttock. If you do not want to gain weight, there are other options. Option number 2: posterior buttock lift. An incision would connect your tummy tuck scar to the top of the buttock crack to the other end of your tummy tuck scar. An incision would be made across the buttock, tissue dissection would be made across the top of the buttock, down the buttock, and the buttock would be pulled up, raised, and made more full. The downside of this is a moderate amount of recovery time and a scar that goes across your buttock. However, in my opinion, this would lead to a much better improvement of a "diaper butt" than a BBL. Option number 3: insertion of a buttock implant. This would add volume to the buttock and correct the inferior portion of the buttock. The problem with buttock implants is that it's a foreign body inside of your buttock. Option number 4: injecting of sculptra. Sculptra is used to manufacture collagen in the body. The creation of collagen leads to buttock augmentation and areas can be targeted on your buttock, especially the inferior portion of your buttock. Therefore, you have several options. In my opinion, your best option would be to either gain weight for a BBL or undergo the posterior body lift with the incision across the buttock.
