Breast implant removal options? 385cc Sientra

Question

I think I have BII. I want to know what options give the best results. I currently have 385cc sientra implants that are 9 years old. Super happy with the size. Don't really want them removed but pretty sure that's what I need to do.

Hello @5 boy mama, thank you for your question. This depends on your goals and expectations There are several ways of perfoming breast implant removal. The most important thing is that you have a total capsulectomy no matter whether the implants are removed en bloc all of the capsule will be removed to avoid future autoinflammatory processes. Depending on the specialist a breast implant removal can be perform through defierent tchniques. My preference is to combine breast implant removal with immediate repositioning of the mammary tissue to address projection of the upper pole and medial areas using the BRALAN Technique (Breast Reconstruction Anatomical Lift Augmentation) since the results will be adequate and have a minimal scar in the shape of an L. I would first explore several doctors to make sure your symptoms are not due to an underlying illness. Your last resort is to remove your implants. Even if you remove them, there is no guarantee that your symptoms will be resolved. If you decide to remove them, I would also remove the capsule. Depending on how much loose skin is present, a breast lift may also be indicated. I'm sorry for your issues.

Removal of breast implants, intact or ruptured, may have positive implications if patients are symptomatic. A constellation of symptoms associated with breast implant illness, and autoimmune disease are considered when treating patients who have breast implants and are concerned about foreign body reaction. In our office, we begin this journey with a consideration of symptoms and exclusion of obvious autoimmune problems. In many of our patients, en bloc removal of breast implants with capsulectomy is important and beneficial. Further clinical evidence about breast implant illness is evolving and will continue to guide treatment. I would suggest removal if there is a possibility of foreign body reaction.

The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

Not all women with BII symptoms improve when their implants and capsules are removed. I would first be tested for food allergies and try to stay away from pesticides and other harmful chemicals. Never use pesticides or insecticides around or in your home. Harmful chemicals are in cosmetics and especially in hair dyes and tattoos and cleaning products. Vinegar and baking soda do great as cleaners and are completely safe. Once you have cleaned up your diet and home and still have symptoms, then consider removing the implants if you want.
