This type of surgery requires specialized skills due to the nature of the procedure, as well as precision when removing the implants, and performing total capsulectomy (the process of completely removing all scar tissue that surrounds an implant). It’s also important to note that not all surgeons offer total capsulectomy during removal; therefore make sure this is part of the consultation discussion prior to making any decisions about moving forward with explant surgery. In the case of severe breast contracture removal of implants is necessary. Providing that patient has enough of breast tissue after removal of implant and portion or entire capsule (if needed) reconstruction can proceed by means of breast lift or breast lift with new implants, this time placed in new pocket. Addition of fat grafts may be needed to achieve desired shape and size. For this complex procedure there is no need for microsurgery expertise since this is needed in those very rare situations when patient own free tissue transfer may be needed to provide sufficient breast volume. These are, however, very long and expensive procedures. Hope this helps.
Dear conniesarah, You require any plastic surgeon who would be able to remove your ruptured implant and do a FULL capsulectomy. Most plastic surgeons in the UK are trained in microsurgery, although microsurgical techniques are not necessary for the type of operation you require. Any implants that have been in for longer than 8 years require a full capsulectomy.A grade IV capsular contracture also requires a capsulectomy. My advice is based on my own practice - I remove a lot of breast implants. I hope this is helpful.
