Based on your goals, I would recommend a breast lift (mastopexy) which would result in a lifted breast with a higher position and reduced size of your nipple-areola. To achieve this, you would need wise-pattern (or anchor) incisions. If you generally like the size of your breasts (ie, do you like they look in clothing in a supportive bra), then you may only need a breast lift/mastopexy (versus a reduction). For these patients, I do a tissue sparing technique, which essentially an auto-augmentation, conserving all of your breast tissue, but lifting the overall breast and bringing the nipple-areola to a higher, more aesthetic position. Depending on your goals and expectations, you have a couple different options.

1. You can do an areola reduction with a circumareolar breast lift. This can reduce the size of your areola and tighten the skin around it. The downside of this procedure is that it can only tighten the breast a minimal amount and the areola can again widen after surgery.

2. You can do a lollipop breast lift. This will create a scar around your areola and a line straight down. This allows for a more aggressive skin tightening of the breast.

3. You can do a wise pattern (anchor scar) breast lift. This will create a scar around your areola, a line straight down, and a line across the fold of your breast. This allows for the most aggressive skin tightening and reshaping.

Hello; Your areolas are large. Your breasts look asymmetrical and somewhat saggy. If you only want to reduce your areolas; periareolar breast lift can be performed. Here, it is necessary to pay attention for the technique and material which is applied. Because, for later, you may encounter with a very large areola again. Breast asymmetry and sagging; cannot be fixed sufficiently; by this technique. If you desire for getting rid of all your problems; I recommend for you; to have a breast lift with the vertical technique.

The excess areola can be removed, and the surrounding skin will be gathered and tucked using a buried permanent suture. This prevents dilation of the areola. Expect a mild lift, and the only scar is around the areolas. Overall you need to balance in your mind the benefit of the areolar reduction on an overall attractive breast, knowing that there will be scar if the areola is reduced. Currently you have natural looking breasts, with larger than desired areolas. Generally this scar fades and is not noticeable in the long term.
