Dr. Leland Deane is a board-certified plastic surgeon based in New York who’s been in practice for over 35 years. Dr. Deane is a senior partner in the New York Plastic Surgical Group, the oldest continuous plastic surgical practice in North America, with locations in Garden City, Manhattan, and Babylon. What a wonderful, thorough and patient doctor. His manners, meticulousness and expertise are none other than perfect. About three years ago I went to DR - what a disaster. I ended up with unnecessary scars that hurt under my arms and implants that caused me pain. After DR I've learned my lesson, and I decided to look none other than to the expert, Dr. Deane. Dr. Deane conducted the corrected surgery and removed the implants and revised the scars and within 2 days I was up and about. There was very little pain and the scars are healing nicely. I hope you're not serious. This is a setup for disaster on so many levels. First off, if one particular surgeon is not good for all these procedures then you are with the wrong person. Based on your photos, I'll give my opinion that you certainly cannot get close to your desired goals based on a single surgery. The breast is 'deflated' and there is a lot of loose skin, with the nipple quite low. You are thin and your muscles are strong, so the idea would be to give you a nice shape as well as fullness. This is difficult to do in a single surgery and time and again we have seen success when the 'lift part' and the 'augment part' are separated. An experienced surgeon can discuss this with you fully and computer imaging will be a big help in communication. Sometimes I have done the augment first and then a lift later if necessary, when it would be much more accurate with minimal scarring.
Absolutely, I would either do nothing or a very conservative upper blepharoplasty. The fullness over your outer upper lid is GOOD and one sign of a youthful looking eye. There is a risk of 'hollowing out' of the upper lid if the bleph is too aggressive. Please see examples on my instagram of good blepharoplasty. Personally, I would would be go to a surgeon who would be very gentle in their approach.

Without preop photos, implant size, it's difficult to tell much, but I see that the scar (presumably at the fold) is fairly fixed and so, yes, the breast seems 'bottomed out' , meaning that there has been over expansion of the skin between the areola and fold of the breast. This will lead to less fullness 'on top'. The answer is not to put in a larger implant necessarily, which may lead to more expansion of the skin. A second opinion may help if your original surgeon seems reluctant to address your concerns. Hope this helps.
This is a great question, and one that I love to answer. As they say, 'it all depends'. For instance, have you had children and when? What do the breasts looks like now? How large were the implants? In general, I have had excellent results after removing implants (and not replacing them). If a subsequent lift/mastopexy is needed, then it becomes much more accurate, with better shape and less scarring, in my opinion. As to the question about being 'smaller' in size, I have found that the change is minimal and certainly worth the improved shape and not having to deal with implants.
